摘　　要：目的 通过对起搏器植入患者随访,了解术后心房高频事件发生情况及可能影响因素。方法 回顾分析2015年1月1日至2016年12月31日在北京大学人民医院心脏中心首次植入起搏器的128例患者,收集年龄、性别、起搏器植入指征、合并疾病、既往房性快速性心律失常病史、心房起搏比例(AP%)、心室起搏比例(VP%)、超声心动图等临床资料,分析比较心房高频事件发生情况。结果 心房高频事件发生率为24.2%,阳性组中年龄、左心房前后径(LAD)明显高于阴性组(P<0.05);阳性组病态窦房结综合症(SSS)比例71%明显高于阴性组43%,(P=0.010);多因素Logistic回归分析显示,年龄(OR=1.049, P=0.024)、LAD(OR=3.329, P=0.001)、SSS(OR=3.823, P=0.006)是心房高频事件发生的独立危险因素。结论 起搏器患者心房高频事件发生率较高,高龄、左心房增大、SSS患者更易发生心房高频事件。Objective To understand the incidence of atrial high rate episodes and possible influencing factors through follow-up of patients with pacemaker implantion.Methods Retrospective analysis of 128 patients who received their first pacemaker implantation at the Heart Center of Peking University People's Hospital from January 1,2015 to December 31,2016.Collect age,gender,pacemaker indication,comorbidities,previous history of atrial tachyarrhythmia,atrial pacing percentage(AP%),ventricular pacing percentage(VP%),echocardiograms clinical data.Analyze and compare the incidence of atrial high rate episodes.Results The incidence of atrial high rate episodes was 24.2%.The age and left atrial diameter(LAD)in the positive group were significantly higher than those in the negative group(P<0.05).The proportion of sick sinus syndrome(SSS)in the positive group(71%)was significantly higher than that in the negative group(43%)(P-0.010).Multivariate Logistic regression analysis showed that age(OR=1.049,P=0.024),LAD(OR=3.329,P=0.001)and SSS(OR=3.823,P-0.006)were independent risk factors for atrial high rate episodes.Conclusions The incidence of atrial high rate episodes in pacemaker patients is higher,and patients with advanced age,left atrial enlargement and SSS are more likely to have atrial high rate episodes.
