摘 要:目的分析母婴血型不合新生儿溶血病患儿检测结果。方法选取信阳市中心医院2021年1月至2023年8月收治的临床高度怀疑新生儿溶血病310例患儿的血样。研究对象均进行血型检测,所有患儿均进行抗体释放试验、游离抗体试验,分析新生儿溶血病患儿血型分布、性别分布、年龄分布、血常规检查结果、抗体释放试验以及游离抗体试验结果。结果A型血患儿新生儿溶血病患病率(81.4%)高于B型血患儿(60.8%)(P<0.05),新生儿溶血病患儿患病率在性别的比较中差异无统计学意义(P>0.05),日龄≤1 d的新生儿患病率最高为84.7%,检验结果双阳占比最高为47.7%,抗体释放试验单阳占比24.2%,新生儿溶血病患儿的总胆红素(188±73)μmol/L、网织红细胞(5.13±2.13)%高于正常组(170±64)μmol/L、(3.82±2.01)%,红细胞计数(5.78±0.24)×10^(12)/L和血红蛋白(163±30)g/L低于正常组(6.24±0.65)×10^(12)/L,(183±28)g/L。结论母婴血型不合新生儿溶血病患儿红细胞计数和血红蛋白降低,总胆红素、网织红细胞增高,A型血患儿高于B型血患儿,随着日龄增高其患病率降低,患病率与性别无关。Objective To analyze of the detection results of neonatal hemolytic disease with maternalinfant blood group incompatibility.Methods The blood samples of 310 children with highly suspected hemolytic disease of newborn treated in Xinyang Central Hospital from January 2021 to August 2023 were selected.All the subjects were tested for blood type,antibody release test and free antibody test were performed to analyze the blood type distribution of hemolytic disease of newborn.The results of sex distribution,age distribution,blood routine test,antibody release test and free antibody test were observed.Results The prevalence rate of hemolytic disease of newborn in children with type A(81.4%)was significantly higher than that in type B(60.8%)(P<0.05).The prevalence of hemolytic disease of newborn was not statistically significant in comparison of gender(P>0.05).The highest prevalence rate of 84.7%in newborns aged≤1 day,the highest proportion of double positive was47.7%,and the positive rate of antibody release test was 24.2%.The total bilirubin(188±73)μmol/L,reticulocyte(5.13±2.13)%were significantly higher in children with hemolytic disease of the newborn than in the normal group(170±64)μmol/L and(3.82±2.01)%,the red blood cell count(5.78±0.24)×10^(12)/L and hemoglobin(163±30)g/L were lower than those in the normal group(6.24±0.65)×10^(12)/L and(183±28)g/L.Conclusion Red blood cell count and hemoglobin decreased and total bilirubin and reticulocyte increased in children with maternal-infant blood group incompatibility.The prevalence rate of type A was higher than that of type B.the prevalence rate decreased with the increase of age,and the prevalence rate had nothing to do with sex.
