孕妇血清人绒毛膜促性腺激素联合经阴道彩色多普勒超声检查对早期异位妊娠的诊断价值

摘　　要：目的探讨孕妇血清人绒毛膜促性腺激素(HCG)联合经阴道彩色多普勒超声(TVCD)检查对早期异位妊娠的诊断价值。方法采取回顾性研究选取2022年1月至2023年1月河南省洛阳市妇幼保健院收治的疑似异位妊娠孕妇98例临床资料作为研究对象,孕妇均接受对症治疗,于入院时检测血清HCG水平并接受TVCD检查,分析孕妇血清HCG水平联合TVCD检查对早期异位妊娠的诊断价值。结果以诊断性刮宫为诊断金标准,根据孕妇病理诊断进行分组,分为异位妊娠组46例、宫内早孕组52例;血清HCG诊断异位妊娠准确度为82%,TVCD检查异位妊娠准确度为84%,血清HCG、TVCD检查联合诊断异位妊娠准确度为96%。异位妊娠组孕妇子宫内膜厚度以及血清HCG水平低于宫内早孕组,差异有统计学意义(P<0.05);孕妇血清HCG水平以及子宫内膜厚度水平单独及联合诊断孕妇早期异位妊娠曲线下面积(AUC)均>0.70,其中联合检测AUC=0.902。结论孕妇血清HCG水平联合TVCD检查对早期异位妊娠诊断价值较高,为临床及早确诊、治疗异位妊娠提供科学依据。Objective To explore the diagnostic value of maternal serum human chorionic gonadotropin(HCG)combined with transvaginal color Doppler ultrasound(TVCD)in early ectopic pregnancy.Methods In the prospective study,a total of 98 pregnant women with suspected ectopic pregnancy admitted to Luoyang Maternal and Child Health Hospital from January 2022 to January 2023 were selected as the research objects.All pregnant women received symptomatic treatment in hospital.Serum HCG level was detected at the time of admission to the hospital and underwent TVCD examination.The diagnostic value of serum HCG level combined with TVCD examination in early ectopic pregnancy was analyzed.Results Diagnostic curettage was used as the gold standard for diagnosis.According to the diagnosis of pregnant women,they were divided into 46 cases of ectopic pregnancy group and 52 cases of intrauterine early pregnancy group.The accuracy of serum HCG in the diagnosis of ectopic pregnancy was 82%,the accuracy of TVCD in the diagnosis of ectopic pregnancy was 84%,and the accuracy of serum HCG combined with TVCD in the diagnosis of ectopic pregnancy was 96%.The endometrial thickness and serum HCG level of pregnant women in the ectopic pregnancy group were lower than those in the intrauterine early pregnancy group,and the differences was statistically significant(P<0.05).The ROC curve showed that the AUC of pregnant women's serum HCG level and the endometrial thickness level of pregnant women's early ectopic pregnancy in single and joint diagnosis was>0.70,and the AUC of the diagnostic value of combined detection was 0.902.Conclusion The serum HCG level of pregnant women combined with TVCD examination has high value in the diagnosis of early ectopic pregnancy,which provides scientific basis for early diagnosis and treatment of ectopic pregnancy.
