尿液干化学法联合尿沉渣镜检法用于白细胞检验的临床价值

摘  要:目的分析临床白细胞检验过程中,使用尿液干化学法+尿沉渣镜检法的临床价值。方法选取2022年3月至2023年8月河南省南阳市第一人民医院收治的122例患者(均接受尿液白细胞检验),所有患者均接受尿液干化学法、尿沉渣镜检法检验,分别记录不同检测方式的诊断敏感度、特异度等指标。结果尿液干化学法+尿沉渣镜检法诊断的敏感度、特异度、阳性预测值、阴性预测值、诊断符合率为:96.4%、92.7%、99.1%、73.3%、95.9%,尿液干化学法诊断的敏感度、特异度、阳性预测值、阴性预测值、诊断符合率为:81.8%、50.0%、93.8%、23.1%、78.1%,联合检测准确率高于单独检测(χ^(2)=11.973、5.042、4.294、9.899、16.296,P<0.01、0.025、0.038、0.002、0.000)。尿液干化学法+尿沉渣的检验准确率95.9%(117例)高于单独的尿液干化学法78.7%(96例)。结论尿液干化学法与尿沉渣镜检能得到更加精准的白细胞检验结果,联合检测比单独尿液干化学法的敏感度、特异度、准确度高,推荐临床检验工作采用联合检验方案。Objective To explore the clinical value of the urine dry chemistry method and the urine sediment microscopy in the process of testing clinical white blood cells.Methods One hundred and twenty-two patients admitted to Nanyang First People's Hospital from March 2022 to August 2023,were randomly selected(all underwent urine leukocyte testing),and all patients underwent urine dry chemistry and urine sediment microscopy tests.The diagnostic sensitivity,specificity,and other indicators of different detection methods were recorded.Results The sensitivity,specificity,positive predictive value,negative predictive value,and diagnostic accuracy of the urine dry chemistry method and urine sediment microscopy were 96.4%,91.7%,99.1%,73.3%,and 95.9%,respectively.The sensitivity,specificity,positive predictive value,negative predictive value,and diagnostic accuracy of the urine dry chemistry method were 81.8%,50.0%,93.8%,23.1%,and 78.7%,respectively.The accuracy of combined detection was significantly higher than that of individual detection(X^(2)=11.973,5.042,4.294,9.899,16.296,P=0.001,0.025,0.038,0.002,<0.01).The accuracy of the urine dry chemistry method combined with urine sediment detection was 117(95.9%)higher than that of the urine dry chemistry method alone,which was 96(78.7%).Conclusion Urine dry chemistry method and urine sediment microscopy can obtain more accurate white blood cell test results,and the combined detection has higher sensitivity,specificity,and accuracy than the urine dry chemistry method alone.It is recommended to use a joint testing plan for clinical testing work.
