交通支静脉结扎联合分段剥脱术与超声引导下泡沫硬化剂治疗下肢隐静脉曲张的效果比较

摘　　要：目的比较交通支静脉结扎联合分段剥脱术与超声引导下泡沫硬化剂治疗下肢大隐静脉曲张的临床疗效。方法选取郑州四六○医院2020年8月至2023年8月就诊的64例下肢大隐静脉曲张患者,根据治疗方法将其分为2组,各32例,对比组给予交通支静脉结扎联合分段剥脱术治疗,观察组在对比组基础上给予超声引导下泡沫硬化剂治疗,比较2组手术指标、静脉疾病临床严重度评分(VCSS)评分、超声指标、血管内皮功能、血清炎症因子和并发症总发生率。结果观察组手术时间、住院时间均短于对比组(P<0.05),观察组术中失血量低于对比组(P<0.05)。观察组治疗后VCSS评分低于对比组(P<0.05)。观察组治疗后反流速度、反流时间低于对比组(P<0.05)。观察组治疗后血清内皮素1(ET-1)水平低于对比组(P<0.05),观察组治疗后血清一氧化氮水平高于对比组(P<0.05)。观察组治疗后血清白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平低于对比组(P<0.05)。观察组并发症总发生率(3%)低于对比组(22%)(P<0.05)。结论超声引导下泡沫硬化剂可有效改善下肢大隐静脉曲张患者临床症状,降低反流速度,缩短反流时间,减轻血管内皮功能受损程度及炎症反应,降低并发症发生率,治疗效果显著,值得借鉴。Objective To compare the clinical efficacy of communicating branch vein ligation combined with fractional exfoliation and ultrasound-guided foam hardener in the treatment of varicose veins of lower extremities.Methods Sixty-four patients with varicose veins of lower extremities treated in Zhengzhou 460 Hospital from August 2020 to August 2023 were selected for the study,and divided into two groups,32 cases in each group.The control group was treated with communicating branch vein ligation and segmental stripping,and the observation group was treated with ultrasound-guided foam hardener treatment on the basis of the comparison group.The surgical indexes,clinical severity score of venous disease(VCSS),ultrasound indexes,vascular endothelial function indexes,serum inflammatory factors indexes,and the total incidence of complications were compared between the two groups.Results The operation time and hospitalization time of the observation group were shorter than that of the control group,P<0.05,and the intraoperative blood loss of the observation group was lower than that of the control group,P<0.05.After treatment,the VCSS score of the observation group was lower than that of the control group,P<0.05.The reflux speed and reflux time in the observation group were lower than those in the control group(P<0.05).After treatment,the serum endothelin-1(ET-1)level of the observation group was lower than that of the control group(P<0.05),and the serum nitric oxide(NO)level of the observation group was higher than that of the control group(P<0.05).After treatment,the serum levels of interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)in the observation group were lower than those in the control group,P<0.05.The total complication rate of the observation group(3%)was lower than that of the control group(22%),P<0.05.Conclusion Ultrasound-guided foam hardener can effectively improve the clinical symptoms of patients with varicose veins of the lower extremities,reduce the reflux speed,shorten the reflux time,reduce the degree
