摘 要:目的探究第三代双源计算机断层扫描(DSCT)结合前瞻性心电图门控高倍率技术在28 d以下危重或复杂先天性心脏病新生儿中的应用效果。方法回顾性收集2013—2021年期间接受心脏计算机断层扫描血管造影(cCTA)的325名先天性心脏病新生儿,评估手术结果与第三代DSCT与经胸超声心动图(TTE)结果之间的诊断一致性,并比较第二代、第三代DSCT扫描的辐射剂量和图像质量。结果DSCT在识别相关的心外血管异常方面优于TTE(敏感性:97.19%vs 80.62%;特异性:99.94%vs 81.89%;阳性预测值:99.48%vs 95.54%;阴性预测值:99.68%vs 97.99%)。第三代DSCT的辐射剂量较第二代明显降低(P<0.05),二者图像质量相近。结论第三代DSCT低管电压结合前瞻性大螺距扫描模式对于评估患有重型或复杂型先天性心脏病患儿优势显著,在不影响图像质量的前提下可降低辐射剂量。Objective To explore the application effect of third-generation dual-source computed tomography(DSCT)combined with prospective electrocardiograph(ECG)-gating high magnification technique in<28 day-old neonates with critical or complex congenital heart disease.Methods A total of 325 neonates with congenital heart disease undergoing cardiac computed tomography angiography(cCTA)were retrospectively collected between between 2013 and 2021.The consistency of diagnostic results between surgery and third-generation DSCT,transthoracic echocardiography(TTE)was evaluated.The radiation doses and images quality of second-generation and third-generation DSCT scans were compared.Results DSCT was superior to TTE in identifying associated extracardiac vascular abnormalities(sensitivity:97.19%vs 80.62%,specificity:99.94%vs 81.89%,positive predictive value:99.48%vs 95.54%,negative predictive value:99.68%vs 97.99%).The radiation dose of third-generation DSCT was significantly lower than that of second-generation DSCT(P<0.05),and the image quality of the two was similar.Conclusion The third-generation DSCT low-tube voltage combined with prospective coarse-pitch scan mode has significant advantages in evaluating neonates with critical or complex congenital heart disease,which can reduce radiation dose without affecting images quality.
