摘  要:目的 探讨CT血管造影(CTA)容积再现影像融合技术应用于腹主动脉瘤腔内修复术(EVAR)的价值。方法 回顾性分析首都医科大学附属北京安贞医院2020年11月至2022年10月行EVAR的60例病人资料,将60例行EVAR的病人按是否使用容积再现技术分组,研究组病人应用容积再现影像融合技术行3D实时路径导航,对照组病人应用传统腔内技术。对两组病人术中数据和复查结果进行对比。结果 60例病人均成功完成手术,影像融合技术在术中实时清晰显示瘤体、瘤颈等三维空间位置关系。研究组病人的术中碘化造影剂用量显著低于对照组,研究组的辐射剂量明显少于对照组,两组间透视时长差异无统计学意义。结论 容积再现影像融合技术可以为EVAR提供介导价值,可以减少术中造影剂用量及辐射剂量。使用容积再现影像融合技术辅助EVAR安全有效。Objective To evaluate the value of CTA volume rendering image fusion in endovascular abdominal aortic repair(EVAR).Methods Retrospective analysis of 60 cases of endovascular aneurysm repair(EVAR) at Beijing Anzhen Hospital,Capital Medical University,from November 2020 to October 2022.60 patients undergoing EVAR were divided into groups according to whether volume rendering technology was applied or not.Patients in the research group applied VR image fusion technology to conduct3D real-time path navigation,while patients in the control group were treated with regular endovascular techniques.Intraoperative data and follow-up results of two groups of patients were compared.Results EVAR was successfully performed in all patients.Image fusion technology can display the three-dimensional spatial position relationship of abdominal aortic aneurysm in real-time during operation.The dosage of iodized contrast agent and radiation dose for operation in the study group was significantly lower than that in the control group,yet there was no significant difference in the fluoroscopy time between the two groups.Conclusion VR image fusion technology can provide a guiding value for EVAR,reducing the amount of contrast agent and radiation dose during operation.Using VR image fusion in EVAR is safe and effective.
