摘 要:目的:使用欧洲医药保健网络(PCNE)分类系统V9.0分析住院缺血性脑卒中患者药物相关问题(DRPs)的特征,并评估临床药师干预的作用。方法:该研究为前瞻性单臂干预试验,受试者为2021年9~10月我院神经内科病区新收治的130例缺血性脑卒中患者。临床药师在多学科诊疗团队(MDT)中提供全程化药物治疗管理(MTM)服务。PCNE分类系统V9.0用于相关DRPs类型、原因、干预措施、接受度和结果的描述。结果:DRPs的发生数为47例,其主要涉及安全性问题(48.94%)和有效性问题(29.79%)。共确定73个DRPs的原因,导致DRPs的主要原因是药物选择(26.03%)和患者本身及其行为不当(23.29%)。根据DRPs的特征,临床药师提供了170项干预措施,169项(99.41%)干预措施被医生或患者接受,共解决44个(93.62%)DRPs。结论:PCNE分类系统具有帮助临床药师有效发现缺血性脑卒中患者的DRPs并帮助实现药学服务数据系统化的收集、分类和评价的潜力,能帮助临床药师更好地提供标准化的MTM服务。Objective:To analyze the characteristics of drug related problems(DRPs)in hospitalized ischemic stroke patients using the Pharmaceutical Care Network Europe(PCNE)classification system V9.0,and evaluate the effects of clinical pharmacists’intervention on DRPs in patients.Methods:This study was a prospective,single-arm interventional trial,involving ischemic stroke patients newly admitted to the Department of Neurology of Taixing People’s Hospital from September to October 2021.Clinical pharmacists provide whole medication therapy management(MTM)services in the multidisciplinary diagnosis and treatment team(MDT).PCNE classification system V9.0 was used to describe the types and causes of DRPs,interventions and their acceptability and results.Results:A total of 130 patients with ischemic stroke were included and 47 DRPs were identified,mainly involving the safety(23,48.94%)and effectiveness(14,29.79%)of treatments.A total of 73 causes of DRPs were identified,and the two most common were drug selection(19,26.03%)and patients themselves and their behaviors(17,23.29%).According to the characteristics of DRPs,clinical pharmacists provided 170 interventions,and 99.41%of them were accepted by doctors or patients.Finally,44 DRPs were solved(93.62%).Conclusion:The PCNE classification system has the potential to assist clinical pharmacists in effectively identifying DRPs in ischemic stroke patients and to facilitate the systematic collection,classification and evaluation of pharmaceutical service data.It can help clinical pharmacists better provide standardized MTM services.
