变应性鼻炎与哮喘的因果关联分析

摘　　要：目的 通过两样本双向的孟德尔随机化研究探讨变应性鼻炎与哮喘的因果关系。方法 利用全基因组关联研究汇总数据,其中变应性鼻炎来自于英国UKB数据库,包括18 934例患者,64 595例对照;哮喘来自于芬兰数据库,包括20 629例患者,135 449例对照。利用逆方差加权(IVW)、MR′Egger、加权中位数和加权模式等方法进行MR分析。采用Cochrane′s Q检验识别MR结果中的异质性。进行MR-Egger截距检验和MR多向性残差和离群值(MR-PRESSO)检测水平多效性。采用留一法检验评估因果关联的可靠性。结果 IVW结果显示,变应性鼻炎与哮喘呈正相关,OR值为5.290(95%CI 2.200~12.716,P<0.001)。Egger-intercept的值为-0.006,不存在水平多效性。留一检验证实了这种关联的可靠性。IVW结果显示,哮喘与变应性鼻炎之间也存在着正相关,OR值为1.051(95%CI 1.031~1.073,P<0.001)。Egger-intercept的值为-0.006,说明不存在水平多效性。留一检验结果显示这种关联的可靠性。结论 变应性鼻炎与哮喘呈正相关,纠正变应性鼻炎对哮喘有一定的预防作用。Objective To investigate the causal relationship between allergic rhinitis and asthma through a two-sample bidi-rectional Mendelian randomization study.Methods The data of allergic rhinitis were collected from the UKB database,including 18934 patients and 64595 controls;the data of Asthma was derived from a Finnish database that included 20629 patients and 135449 controls.Inverse variance weighting(IVW),MR′Egger,weighted median and weighted model are used for MR Analysis.Heterogenei-ty in MR.Results was identified by Cochrane′s Q test.The MR-Eggerintercepttestand Mendelian randomization pleiotropy residual sum and outlier(MR-PRESSO)were performed to detect the level of pleipotency.The reliability of causal association was evaluated by the leave-one-out test.Results The IVW results showed that allergic rhinitis was positively correlated with asthma with an OR value of 5.290(95%CI 2.200-12.716,P<0.001).The value of Egger intercept is-0.006,indicating no horizontal pleiotropy.The leave-one-out test confirmed the reliability of this association.The IVW results also showed that asthmawas positively correlated withallergic rhinitis,with an OR value of 1.051(95%CI 1.031-1.073,P<0.001).The value of Egger intercept is-0.006,indicating no hori-zontal pleiotropy.The leave-one-out test results show the reliability of this association.Conclusion Allergic rhinitis is positively correlated with asthma.Correcting allergic rhinitis can prevent asthma to some extent.
