摘　　要：目的评价多柔比星脂质体在多种恶性肿瘤治疗中的安全性。方法系统检索中国期刊全文数据库、万方数据知识服务平台、维普资讯、PubMed、Embase、Medline、the Cochrane Library等数据库,按照纳入和排除标准筛选2012年1月至2023年2月有关多柔比星脂质体与传统化疗药物治疗恶性肿瘤的安全性随机对照研究,采用RevMan 5.3软件进行各效应指标的meta分析。结果最终纳入中英文RCTs文献27篇,涉及3705例肿瘤患者。meta分析结果显示,多柔比星脂质体治疗的观察组血小板减少、WBC减少、贫血、外周神经毒性、胃肠道反应、心脏毒性6类不良反应发生率相较于传统化疗药物治疗的对照组有更好的安全性,差异均有统计学意义(P<0.05)。以血小板减少、WBC减少、贫血、粒细胞减少和腹泻发生率为指标绘制的漏斗图形均对称分布,提示纳入文献的发表偏倚较小。结论多柔比星脂质体在多种恶性肿瘤治疗的安全性明显优于传统化疗药物治疗。Objective To evaluate the safety of liposomal doxorubicin in the treatment of various malignant tumors.Methods CNKI,Wanfang Data,VIP Information,PubMed,Embase,Medline,the Cochrane Library,and other databases were systematically searched to screen randomized controlled studies on the safety of doxorubicin liposomes and traditional chemotherapy drugs in the treatment of malignant tumors from January 2012 to February 2023.Meta-analysis of each effect index was performed using RevMan 5.3 software.Results A total of 27 articles on RCTs in both Chinese and English were included,involving 3705 cancer patients.Meta-analysis results showed that for thrombocytopenia,WBC reduction,anemia,peripheral nerve toxicity,gastrointestinal reaction,cardiotoxicity,liposomal doxorubicin treatment group had better safety compared with the traditional chemotherapy drugs,and the differences were statistically significant(P<0.05).The funnel plots plotted with thrombocytopenia,WBC reduction,anemia,granulocytopenia and diarrhea incidence as indicators ware symmetrically distributed,indicating a small publication bias in the included literature.Conclusion The safety of liposomal doxorubicin in the treatment of various malignant tumors is significantly better than that of conventional chemotherapy drugs.
