胆宁片治疗老年慢传输型便秘的临床疗效研究

摘　　要：目的探讨胆宁片治疗老年慢传输型便秘的临床疗效。方法前瞻性选取2022年1月至2023年5月鄂州市中心医院收治的90例老年慢传输型便秘患者为研究对象,按照随机数字表法分为对照组和观察组,各45例。对照组规律饮食,同时对排便行为进行培养,给予枸橼酸莫沙必利分散片治疗,观察组在对照组基础上给予胆宁片治疗,比较两组患者的肠道菌群丰度、中医症候积分、肠神经递质、结肠传输功能、胃肠激素水平、便秘主要症状评分及临床疗效。结果治疗后,两组患者的双歧杆菌、乳酸杆菌、P物质、钡剂排空率、胃泌素和胃动素水平均较治疗前升高,且观察组各指标均高于对照组;两组患者的肠杆菌、梭杆菌、中医症候积分和生长抑素、血管活性肠肽、一氧化氮水平及便秘主要症状评分均较治疗前降低,且观察组各指标均低于对照组,差异有统计学异议(P<0.05)。观察组治疗总有效率高于对照组(93.33%比77.78%),差异有统计学异议(P<0.05)。结论胆宁片可显著改善老年慢传输型便秘患者的肠道菌群平衡、肠神经递质和胃肠激素水平,还可提高结肠传输功能,改善临床症状。Objective To investigate the clinical efficacy of danning tablets in the treatment of slow-transit constipation in elderly patients.Methods A prospective study was conducted in 90 elderly patients with slow transit constipation admitted to Ezhou Central Hospital from January 2022 to May 2023.They were randomly divided into a control group and an observation group,with 45 patients in each group.The control group had a regular diet and defecation behavior was cultivated.Mosapride citrate dispersible tablets were administered to the observation group.Based on the control group,danning tablets were given to the observation group.The abundance of intestinal microbiota,traditional Chinese medicine symptom scores,intestinal neurotransmitters,colonic transmission function,gastrointestinal hormone levels,main symptom scores of constipation,and clinical efficacy of the two groups of patients were compared.Results After treatment,the levels of bifidobacteria,lactobacilli,substance P,barium emptying rate,gastrin and motilin in both groups of patients increased compared to before treatment,and all indicators in the observation group were higher than those in the control group;the levels of enterobacterium,fusobacterium,traditional Chinese medicine syndrome score,somatostatin,vasoactive intestinal peptide,nitric oxide,and major constipation these of symptom scores of the two groups of patients were lower than before treatment,and the observation group had lower indicators than those of the control group,with statistically significant differences(P<0.05).The total effective rate of the observation group was higher than that of the control group(93.33%vs 77.78%),with statistically significant differences(P<0.05).Conclusion Danning tablets can significantly improve the balance of intestinal microbiota,levels of intestinal neurotransmitters and gastrointestinal hormones in elderly patients with slow transit constipation.They can also improve colon transit function and improve clinical symptoms.
