摘  要:目的探究血清微小RNA 21(miR-21)、转化生长因子β1(TGF-β1)、二肽基肽酶-4(DPP4,又称CD26)在瘢痕疙瘩组织中的表达情况和相关性。方法选取2020年3月至2023年3月河北医科大学第四医院收治的82例行瘢痕疙瘩组织切除术患者的术后组织作为研究组,另选取同期年龄、性别、BMI相匹配的56例受试者的正常皮肤组织作为对照组。比较两组的一般资料;比较两组组织中的miR-21、TGF-β1、CD26水平。采用多因素logistic回归分析瘢痕疙瘩组织生成的影响因素;采用Pearson相关性分析miR-21、TGF-β1、CD26与瘢痕疙瘩组织发生的相关性;并通过绘制受试者工作特征(ROC)曲线分析miR-21、TGF-β1、CD26对瘢痕疙瘩组织的诊断价值。结果两组的工作类型、民族、miR-21、TGF-β1、CD26水平进行比较,差异有统计学意义(t=5.971、6.012、4.151、3.564、5.735,P<0.05)。多因素logistic回归分析结果显示,工作类型、miR-21、TGF-β1、CD26水平均是瘢痕疙瘩组织发生的影响因素(P<0.05)。Pearson相关性分析结果显示,miR-21、TGF-β1、CD26与瘢痕疙瘩组织发生具有正相关性(r=0.64、0.58、0.71,P<0.05)。ROC曲线结果显示,miR-21、TGF-β1、CD26及三者联合的AUC面积分别为0.714、0.682、0.768、0.825,这表明三者对瘢痕疙瘩组织发生均具有一定的诊断价值,且三者联合效果最佳(P<0.05)。结论miR-21、TGF-β1、CD26在瘢痕疙瘩组织中呈高表达状态,三者与瘢痕疙瘩组织的发生呈正相关性,且三者对于瘢痕疙瘩组织的发生均具有一定的诊断价值。Objective To investigate the expression and correlation of serum micrornas 21(miR-21),transforming growth factorβ1(TGF-β1)and dipeptidyl peptidase-4(DPP4,also known as CD26)in keloid tissues.Methods The postoperative tissues of 82 patients undergoing keloidectomy admitted to the Fourth Hospital of Hebei Medical University from March 2020 to March 2023 were retrospectively selected as the study group,and the normal skin tissues of 56 subjects matching age,gender and BMI during the same period were selected as the control group.The general data of the two groups were compared.The levels of miR-21,TGF-β1 and CD26 ibetween the two groups were compared.Multivariate logistic regression was used to analyze the influencing factors of keloid formation.Pearson correlation was used to analyze the correlation between miR-21,TGF-β1,CD26 and keloid tissue development.The diagnostic value of miR-21,TGF-β1 and CD26 in keloid tissue was analyzed by drawing the receiver operating characteristic(ROC)curve.Results The work type,miR-21,TGF-β1 and CD26 levels of the two groups were compared,and the differences were statistically significant(t=5.971、6.012、4.151、3.564、5.735,P<0.05).Multivariate logistic regression analysis showed that job type,ethnicity,miR-21,TGF-β1 and CD26 levels were all influential factors in the development of keloid tissue(P<0.05).Pearson correlation analysis showed that miR-21,TGF-β1 and CD26 were positively correlated with keloid histogenesis(r=0.64,0.58,0.71,P<0.05).The ROC curve results showed that the AUC area of miR-21,TGF-β1,CD26 and their combination were 0.714,0.682,0.768 and 0.825,respectively,indicating that the three had certain diagnostic value for keloid histogenesis,and the combined effect of the three was the best(P<0.05).Conclusion miR-21,TGF-β1 and CD26 are highly expressed in keloid tissue,and the three are positively correlated with the occurrence of keloid tissue,and all of them have certain diagnostic value for the occurrence of keloid tissue.
