血清D-D、PCT联合CRP检测在肺炎支原体肺炎患儿病情及预后评估中的作用

摘　　要：目的分析血清D-二聚体(D-D)、降钙素原(PCT)联合C反应蛋白(CRP)检测在肺炎支原体肺炎(MPP)患儿病情及预后评估中的作用。方法选取2021年9月至2022年5月宣城市人民医院收治的MPP患儿151例为研究对象,根据病情严重程度分为轻症组104例及重症组47例,比较不同病情严重程度患儿血清D-D、PCT及CRP水平。分析血清D-D、PCT及CRP水平与CPIS评分的相关性。比较不同预后患儿血清D-D、PCT及CRP水平,分析D-D、PCT及CRP单一、联合对MMP患儿预后的评估价值。结果重症组血清D-D、PCT、CRP水平及CPIS评分均显著高于轻症组,差异具有统计学意义(P<0.05)。血清D-D、PCT、CRP水平均与CPIS评分呈正相关(P<0.05)。预后不良组血清D-D、PCT及CRP水平均显著高于预后良好组,差异具有统计学意义(P<0.05)。ROC结果显示,D-D+PCT+CRP检查灵敏度为0.932,特异度为0.901,AUC=0.856(95%CI:0.846~0.935),均高于D-D、PCT及CRP单一检查(P<0.05)。结论血清D-D、PCT及CRP与MPP病情进展存在密切联系,可作为预后评估的重要指标,三者联合更有利于判断MPP患儿的病情严重程度及预后情况。Objective To analyze the effects of serum D-dimer(D-D),procalcitonin(PCT)combined with C-reactive protein(CRP)in the assessment of disease condition and prognosis of children with mycoplasma pneumoniae pneumonia(MPP).Methods A total of 151 children with MPP admitted to Xuancheng People's Hospital from September 2021 to May 2022 were selected as the study objects and were divided into the mild group(104 cases)and the severe group(47 cases)according to the severity of the disease.The levels of serum D-D,PCT and CRP in children with different severity of the disease were compared.The correlation between serum D-D,PCT and CRP levels and CPIS score was analyzed.The levels of serum D-D,PCT and CRP in different prognosis of children were compared,and the value of single or combined D-D,PCT and CRP in the prognosis of MMP children was analyzed.Results The levels of D-D,PCT,CRP and CPIS in the severe group were significantly higher than those in the mild group,and the difference was statistically significant(P<0.05).Serum D-D,PCT and CRP levels were positively correlated with CPIS score(P<0.05).Serum D-D,PCT and CRP levels in the poor prognosis group were significantly higher than those in the good prognosis group,and the difference was statistically significant(P<0.05).The ROC results showed that the sensitivity and specificity of D-D+PCT+CRP were 0.932 and 0.901 respectively,the AUC=0.856(95%CI:0.846~0.935),which was higher than that of D-D,PCT and CRP alone(P<0.05).Conclusion Serum D-D,PCT and CRP are closely related to the progression of MPP and can be used as important indica-tors for prognosis assessment.The combination of the three is more conducive to determining the severity and prognosis of children with MPP.
