摘　　要：目的 探讨Er,Cr:YSGG激光联合种植体打磨抛光治疗种植体周围炎的临床疗效。方法 选择2020年6月至2022年5月,种植体周围炎患者84例,按照随机数字表法分为实验组和对照组,每组患者42例。对照组给予种植体打磨抛光、龈下刮治;实验组给予Er,Cr:YSGG激光联合种植体打磨抛光、龈下刮治,观察两组治疗后3个月的临床疗效。比较两组治疗前和治疗后3个月牙周指标包括菌斑指数(plaque index, PLI)、龈沟出血指数(sulcus bleeding index,SBI)、探诊深度(probing depth, PD)、临床附着水平(clinical attachment level, CAL),龈沟液炎症因子包括龈沟液白细胞介素-1β(interleukin-1β, IL-1β)、白细胞介素-6(interleukin-6, IL-6)、转化生长因子β1 (transforming growth factor-β1, TGF-β1)、基质金属蛋白酶-8(matrix metalloproteinase 8, MMP-8)水平改变。采用健康状况调查简表(The medical outcomes study 36-item short from health survey, SF-36)评分及视觉模拟评分(visual analogue scale, VAS)评估患者生活质量及疼痛情况。结果 治疗后3个月,实验组总有效率(95.2%)高于对照组(81.0%)(P<0.05)。治疗后3个月,两组PLI、SBI、PD、CAL、IL-1β、IL-6、TGF-β1、MMP-8较治疗前降低(P<0.05),实验组PLI、SBI、PD、CAL、IL-6、IL-1β、TGF-β1、MMP-8显著低于对照组(P<0.05)。两组患者治疗后3个月SF-36评分均较治疗前显著升高,VAS均较治疗前显著降低(P<0.05);实验组SF-36评分显著高于对照组,VAS评分显著低于对照组(P<0.05)。结论 Er,Cr:YSGG激光联合种植体打磨抛光治疗种植体周围炎可以有效改善患者牙周指标,降低龈沟液炎性因子水平和疼痛程度。Objective To investigate the clinical efficacy of Er,Cr:YSGG laser combined with implant polishing in the treatment of peri-implantitis.Methods Totally 84 patients with peri-implantitis treated over the period from Jun.2020 to May 2022 were selected and divided randomly into an experimental group and a control group,42 patients in each group.The patients in the control group were treated with implant polishing and subgingival scraping,and those in the experimental group with Er,Cr:YSGG laser in addition.The clinical efficacy of the two groups was observed 3 months after the treatment.The changes to the periodontal indexes including plaque index(PLI),sulcus bleeding index(SBI),probing depth(PD),clinical attachment level(CAL)and inflammatory factors,namely,gingival crevicular fluid including interleukin-1β(IL-1β),interleukin-6(IL-6),transforming growth factor-β1(TGF-β1)and matrix metalloproteinase 8(MMP-8),were compared between the two groups respectively before and 3 months after treatment.The medical outcomes study 36-item short from health survey(SF-36)and visual analog scale(VAS)were used to evaluate patients'quality of life and pain status.Results Three months after the treatment,the total effective rate(95.2%)in the experimental group was higher than that(81.0%)in the control group(P<0.05).For both groups,the PLI,SBI,PD,CAL,IL-1β,IL-6,TGF-β1 and MMP-8 were lower than those before the treatment.The PLI,SBI,PD,CAL,IL-6,IL-1β,TGF-β1 and MMP-8 in gingival crevicular fluid of the experimental group were significantly lower than those in the control group(P<0.05).The SF-36 scores of both groups were significantly higher than those before the treatment,and the VAS scores remarkably lower than those before the treatment(P<0.05).The SF-36 score was much higher in the experimental group than in the control group,and the VAS score distinctively lower than in the control group(P<0.05).Conclusions As to the treatment of peri-implantitis,the combination of Er,Cr:YSGG laser and implant polishing will effectively imp
