多维度强化护理结合阶段性功能康复训练对老年股骨颈骨折患者运动功能、自护能力及疼痛程度的影响

摘　　要：目的探讨多维度强化护理结合阶段性功能康复训练对老年股骨颈骨折患者运动功能、自护能力及疼痛程度的影响。方法本研究为回顾性分析,将在山东大学齐鲁医院于2022年1—12月行全髋关节置换术的67例老年股骨颈骨折患者纳入研究,采用随机数字表法分为两组,对照组33例采用常规护理,观察组34例采用多维度强化护理结合阶段性功能康复训练。比较两组干预前后的运动功能、自护能力、疼痛程度、疾病认知情况和干预后一个月的深静脉血栓发生率。结果干预后1个月,观察组Fugl-Meyer运动功能评分量表(FMA)、自我护理能力测定量表(ESCA)评分均高于对照组(均P<0.05)。干预后1个月,观察组视觉模拟量表(VAS)评分、深静脉血栓发生率均低于对照组(均P<0.05)。干预后1个月,观察组治疗控制、病程、疾病影响、个人控制、症状鉴别、情绪反应、关注、疾病理解8个条目评分均低于对照组(均P<0.05)。结论对于接受全髋关节置换术的老年股骨颈骨折患者采用多维度强化护理结合阶段性功能康复训练有助于改善患者的肢体运动功能、自护能力,减轻疼痛程度,降低深静脉血栓发生率。Objective To investigate the effects of multi-dimensional intensive nursing combined with phased functional rehabilitation training on motor function,self-care ability and pain degree in elderly patients with femoral neck fracture.Methods This study is a retrospective analysis,67 cases of elderly patients with femoral neck fracture who underwent total hip arthroplasty at Qilu Hospital of Shandong University from January to December 2022 were included in the study,and were divided into two groups by using the randomized numerical table method,33 cases in the control group used conventional nursing care,and 34 cases in the observation group used multidimensional intensive nursing care combined with stage-by-stage functional rehabilitation training.The motor function,self-care ability,pain level and incidence of deep vein thrombosis one month after intervention were compared between the two groups before and after intervention.Results At 1 month post-intervention,the Fugl-Meyer assessment scale(FMA)and exercise of self-care agency scale(ESCA)scores of the observation group were higher than those of the control group(all P<0.05);At 1 month after the intervention,the visual analogue scale(VAS)score,disease awareness situation and the incidence of deep vein thrombosis in the observation group were lower than those in the control group(all P<0.05).At 1 month after the intervention,the scores of 8 items of treatment control,disease course,disease impact,personal control,symptom identification,emotional response,concern and disease understanding in the observation group were lower than those in the control group,and the difference was statistically significant(all P<0.05).Conclusion For elderly patients with femoral neck fracture who have undergone total hip replacement,multi-dimensional intensive nursing combined with phased functional rehabilitation training can help improve limb motor function,self-protection ability and reduce pain,to reduce the incidence of deep vein thrombosis.
