文质金老中医运用参苓白术散加减轻恶性肿瘤化疗毒副反应的临床研究

摘　　要：目的探讨文质金老中医运用参苓白术散加减轻恶性肿瘤化疗毒副反应的临床效果。方法采用随机数字表法将2021年6月-2023年7月会昌县中医院文质金诊室收治的86例恶性肿瘤患者分为两组,每组各43例。对照组单纯采用化疗治疗,观察组加用参苓白术散加减治疗,治疗21d后,对两组患者毒副反应、免疫功能和生活质量进行评估。结果观察组恶心呕吐、腹泻、食欲不振、骨髓抑制轻于对照组(P<0.05);观察组治疗后CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)高于对照组,CD8^(+)低于对照组(P<0.05);观察组治疗前后Karnofsky评分比较,没有统计学意义(P>0.05);治疗后对照组Karnofsky评分低于治疗前,且观察组高于对照组(P<0.05)。结论参苓白术散加减方是文质金老中医根据经方参苓白术散、个人经验总结的经验方,具有补益脾胃之效,“脾为后天之本”,且“脾为中土,土为万物之本,故运行水谷,化津液以灌溉于肝心肺肾之四脏者也”,为生命活动力的大后方,因此顾护恶性肿瘤患者的脾胃功能,可减轻其化疗毒副反应,增强其免疫功能,且不会降低患者生活质量,安全性好,值得临床推广应用。Objective To explore the clinical effect of using Shenling Baizhu Powder in combination with Wen Zhijin,a veteran practioner of TCM,to reduce the toxic side effects of chemotherapy for malignant tumors.Method 86 malignant tumor patients admitted to our Wen Zhijin Clinic from June 2021 to July 2023 were divided into two groups using a random number table method,with 43 patients in each group.The control group was treated with chemotherapy alone,while the observation group was treated with modified Shenling Baizhu Powder.After 21 days of treatment,the toxic side effects,immune function,and quality of life of the two groups of patients were evaluated.Results Nausea,vomiting,diarrhea,loss of appetite,and bone marrow suppression were milder in the observation group than in the control group(P<0.05);after treatment,the CD3^(+),CD4^(+),CD4^(+)/CD8^(+)levels in the observation group were higher than those in the control group,while CD8^(+)levels were lower than those in the control group(P<0.05);comparison of Karnofsky scores before and after treatment in the observation group(P>0.05);after treatment,the Karnofsky score in the control group was lower than before treatment,and the observation group was higher than the control group(P<0.05).Conclusion The modified formula of Shenling Baizhu Powder is an empirical formula summarized by Wen Zhijin,a veteran practioner of TCM,based on the classic formula and personal experience.It has the effect of tonifying the spleen and stomach."The spleen is the foundation of the acquired world"and"the spleen is the middle earth,and the earth is the foundation of all things.Therefore,it runs water and grains,transforms body fluids to irrigate the four organs of the liver,heart,lungs,and kidneys."It is the main source of life activity.Therefore,taking care of the spleen and stomach function of malignant tumor patients can reduce their chemotherapy toxicity and side effects,Enhance its immune function without reducing the patient's quality of life,with good safety,and worthy of clinical pro
