摘 要:目的分析癫痫患儿外周血单个核细胞微小RNA-146a(miR-146a)、多药耐药相关蛋白1(MRP1)表达与左乙拉西坦治疗疗效的相关性。方法选取2021年1月-2023年1月衡阳市妇幼保健院收治的50例癫痫患儿为研究对象,纳入癫痫组,另选取同期50例体检健康幼儿为对照组,均采用聚合酶链式反应(PCR)检测外周血单个核细胞miR-146a、MRP1表达;不同发作频率、严重程度癫痫患儿外周血单个核细胞miR-146a、MRP1表达有差异;所有癫痫患儿均接受左乙拉西坦治疗,根据治疗结果分为治疗有效组(n=40)和治疗无效组(n=10),比较两组外周血单个核细胞miR-146a、MRP1表达,并采用Pearson相关性分析对上述指标表达水平与左乙拉西坦治疗疗效的相关性进行分析。结果癫痫组miR-146a、MRP1相对表达量均高于对照组(P<0.05)。频繁发作组miR-146a、MRP1相对表达量均高于单次发作组(P<0.05)。重度癫痫患儿miR-146a、MRP1相对表达量均高于中度组和轻度组(P<0.05)。治疗有效组miR-146a、MRP1相对表达量均低于治疗无效组(P<0.05)。Pearson相关性分析显示,miR-146a、MRP1相对表达量与左乙拉西坦治疗疗效均呈负相关(r=-0.483、-0.520,P<0.05)。结论癫痫患儿外周血单个核细胞miR-146a、MRP1呈高水平表达,且与左乙拉西坦治疗疗效呈负相关,可作为癫痫治疗疗效评估的有效指标。Objective To analyze the correlation between the expression of small RNA-146a(miR-146a)and multidrug resistancerelated protein 1(MRP1)in peripheral blood mononuclear cells and the efficacy of levetiracetam in children with epilepsy.Methods 50 children with epilepsy admitted from January 2021 to January 2023 were selected as the study subjects inclusion in the epilepsy group,in addition,50 healthy children were selected as the control group,Polymerase chain reaction(PCR)was used to detect the expression of miR146a and MRP1 in peripheral blood mononuclear cells;differential expression of miR-146a and MRP1 in peripheral blood mononuclear cells of epileptic children with different seizure frequency and severity;all the children with epilepsy were treated with levetiracetam,according to the treatment results,it was divided into treatment effective group(n=40)and treatment ineffective group(n=10),to compare the expression of miR-146a and MRP1 in peripheral blood mononuclear cells,and Pearson correlation analysis analyze the correlation between the expression level of the above indicators and the efficacy of levetiracetam.Results The relative expression level of miR-146a and MRP1 in the epilepsy group was higher than that in the control group(P<0.05).The relative expression level of miR-146a and MRP1 was higher in the frequent attacks group than in the single episode group(P<0.05).The relative expression level of miR-146a and MRP1 in children with severe epilepsy was higher than that in the moderate and mild groups(P<0.05).The relative expression level of miR-146a and MRP1 in the treatment effective group was lower than that in the treatment ineffective group(P<0.05).Pearson Correlation analysis showed that the relative expression of miR-146a and MRP1 was negatively correlated with the therapeutic efficacy of levetiracetam(r=-0.483,-0.520,P<0.05).Conclusion The peripheral blood mononuclear cells miR-146a and MRP1 were expressed at high levels,and they were negatively correlated with the efficacy of levetiracetam,which c
