补肾活血汤联合经尿道前列腺等离子电切术治疗重度前列腺增生的疗效及对患者术后膀胱动力学指标的影响

摘　　要：目的探讨补肾活血汤联合经尿道前列腺等离子电切术(TURP)治疗重度前列腺增生的疗效及对患者术后膀胱动力学指标的影响。方法选取2021年1月-2023年1月衡阳市中医医院收治的重度前列腺增生患者68例,遵循随机数字原则分为两组,每组各34例。两组均行TURP,在此基础上对照组采用常规治疗,观察组联合补肾活血汤治疗。比较两组临床疗效,治疗前后膀胱动力学指标[前列腺体积(PV)、膀胱残余尿量(RV)、最大尿流率(Qmax)],性激素指标[黄体生成素(LH)、雌二醇(E2)、睾酮(T)]水平变化及不良反应发生率。结果观察组总有效率为91.18%,高于对照组的70.59%(P<0.05)。治疗后,观察组膀胱动力学指标PV、RV水平均低于对照组,Qmax水平高于对照组(P<0.05)。治疗后,观察组性激素指标LH、E2水平均低于对照组,T水平高于对照组(P<0.05)。观察组不良反应发生率为8.82%,低于对照组的29.41%(P<0.05)。结论补肾活血汤联合TURP治疗重度前列腺增生疗效显著,能有效改善患者膀胱动力学,促进性激素水平平衡,且安全性良好。Objective To investigate the efficacy of tonifying kidney and promoting blood circulation soup combined with transurethral prostate plasma resection(TURP)in the treatment of severe prostatic hyperplasia and its effect on postoperative bladder dynamics in patients.Methods A total of 68 patients with severe prostatic hyperplasia admitted to our hospital from January 2021 to January 2023 were divided into two groups according to the principle of random numbers,with 34 patients in each group.Both groups underwent TURP,on which the control group used conventional treatment,and the observation group was combined with tonifying kidney and promoting blood circulation soup.Compare the clinical efficacy of the two groups,bladder kinetic indicators[prostate volume(PV),bladder residual urine volume(RV),maximum urinary flow rate(Qmax)],sex hormone indicators[yellow hormone(LH),estradiol(E2),testosterone(T)],and the incidence of adverse reactions.Results The total response rate in the observation group was 91.18%,which was higher than 70.59%in the control group(P<0.05).After treatment,the PV and RV levels of the observation group were lower than those of the control group,and the Qmax level was high compared with the control group(P<0.05).After treatment,the LH and E2 levels were lower than those of the control group,and the T level was high than the control group(P<0.05).The incidence of adverse reactions in the observation group was 8.82%,lower than 29.41%in the control group(P<0.05).Conclusion The treatment of tonifying kidney and promoting blood circulation soup combined with TURP is effective,which can effectively improve bladder dynamics,promote the balance of sex hormone levels,and have good safety.
