研究心脏彩色多普勒超声在高血压左心室肥大伴左心衰竭中的应用价值

摘　　要：目的探讨采用心脏彩色多普勒超声检查高血压左心室肥大合并左心衰竭的诊断效果情况。方法随机选取2020年2月—2023年5月南京市高淳中医院进行诊断的70例高血压左心室肥大合并左心衰竭患者为研究组;同时间段选取70例健康体检者为参照组;对于两组研究对象合理展开心脏彩色多普勒超声诊断,并且就心功能不同分级患者的结果展开对比。结果研究组的左室收缩末期内径(left ventricular end systolic diameter,LVESD)、左室舒张末期内径(left ventricular end diastolic diameter,LVEDD)、左心房内径(internal diameter of left atrium,LAD)以及E/EA高于参照组,差异有统计学意义(P<0.05);研究组的左心室射血分数(left ventricle ejection fraction,LVEF)(56.35±5.19)%低于参照组,差异有统计学意义(t=10.621,P<0.05)。Ⅲ级患者的LVESD、LVEDD、LAD以及E/EA高于Ⅰ级、Ⅱ级患者,差异有统计学意义(P<0.05);Ⅲ级患者的LVEF低于Ⅰ级、Ⅱ级患者,差异有统计学意义(P<0.05)。结论在诊断高血压左心室肥大合并左心衰竭患者中采用心脏彩色多普勒超声方法的效果较好,可对患者的心脏结构清晰显示,对患者的心脏功能可以进行有效评估。Objective Exploring the diagnostic efficacy of using color Doppler echocardiography to examine hypertensive left ventricular hypertrophy combined with left heart failure.Methods A total of 70 patients with hypertensive left ventricular hypertrophy and left heart failure diagnosed in Nanjing Gaochun Traditional Chinese Medicine Hospital from February 2020 to May 2023 were randomly selected as the study group.At the same time,70 healthy subjects were selected as the reference group.For the two groups of subjects,the diagnosis of cardiac color Doppler ultrasound was reasonably carried out,and the results of patients with different grades of cardiac function were compared.Results The left ventricular end systolic diameter(LVESD),left ventricular end diastolic diameter(LVEDD),internal diameter of left atrium(LAD)and E/EA in the study group were higher than those in the reference group,and the differences were statistically significant(P<0.05).The left ventricular ejection fraction(LVEF)(56.35±5.19)%of the study group was lower than that of the reference group,and the difference was statistically significant(t=10.621,P<0.05).LVESD,LVEDD,LAD and E/EA in patients with gradeⅢwere higher than those in patients with gradeⅠandⅡ,and the differences were statistically significant(P<0.05).The LVEF of patients with gradeⅢwas lower than that of patients with gradeⅠandⅡ,and the difference was statistically significant(P<0.05).Conclusion The use of color Doppler echocardiography in the diagnosis of hypertensive patients with left ventricular hypertrophy and left heart failure is more effective,as it can clearly display the patient's cardiac structure and effectively evaluate their cardiac function.
