比较股骨颈骨折采用X射线片、CT及MRI诊断的价值

摘　　要：目的评价X射线片、CT诊断、MRI诊断在股骨颈骨折临床诊断中的使用效能。方法便利选取2022年10月—2023年7月菏泽市中医医院收治的83例疑似股骨颈骨折患者作为研究对象,全部均给予X射线片、CT诊断、MRI诊断,手术结果为金标准分析3种诊断方式的效能。结果手术结果阳性检出率为89.16%,X射线片阳性检出率为61.45%,CT诊断阳性检出率为74.70%,MRI诊断阳性检出率为87.95%。CT诊断敏感度和准确度高于X射线片,差异有统计学意义(P<0.05)。X射线片和CT诊断特异度比较,差异无统计学意义(P>0.05)。MRI诊断敏感度、特异度、准确度均高于X射线片,差异有统计学意义(P<0.05)。MRI诊断敏感度97.30%、特异度88.89%、准确度96.39%均高于CT诊断的77.02%、44.44%、73.49%,差异有统计学意义(P<0.05)。结论MRI诊断在股骨颈骨折中的应用价值较高,相比于X射线片、CT诊断,MRI诊断的准确度显著高于其他两种诊断方式,具有较高的敏感度、特异度、准确度。Objective To evaluate the efficacy of X-ray,CT and MRI in the clinical diagnosis of femoral neck fracture.Methods A total of 83 patients with suspected femoral neck fracture admitted to Heze Traditional Chinese Medicine Hospital from October 2022 to July 2023 were selected as the study objects conveniently.All of them were given X-ray,CT and MRI diagnosis.The surgical results were the gold standard to analyze the efficacy of the three diagnostic methods.Results The positive detection rate of operation results was 89.16%,X-ray positive detection rate was 61.45%,CT positive detection rate was 74.70%,MRI positive detection rate was 87.95%.The diagnostic sensitivity and accuracy of CT were higher than that of X-ray,and the difference was statistically significant(P<0.05).There was no significant difference in diagnostic specificity between X-ray and CT(P>0.05).The diagnostic sensitivity,specificity and accuracy of MRI were higher than those of X-ray film,and the difference was statistically significant(P<0.05).The diagnostic sensitivity,specificity and accuracy of MRI were 97.30%,88.89%and 96.39%higher than those of CT(77.02%,44.44%and 73.49%),and the differences were statistically significant(P<0.05).Conclusion MRI diagnosis has high application value in femoral neck fracture.Compared with X-ray film and CT diagnosis,MRI diagnosis accuracy is significantly higher than the other two diagnostic methods,with higher sensitivity,specificity and accuracy.
