依那普利联合氨氯地平用于原发性高血压治疗的临床价值研究

摘　　要：目的分析老年冠状动脉粥样硬化性心脏病(简称冠心病)治疗中螺内酯联合瑞舒伐他汀钙的疗效。方法方便选取2022年8月—2023年8月就诊于陕西省榆林市佳县人民医院的68例老年冠心病患者为研究对象,根据随机数表法为对照组和观察组,各34例,其中对照组单纯应用瑞舒伐他汀钙治疗,观察组应用螺内酯联合瑞舒伐他汀钙治疗,对比两组治疗前后临床疗效、心功能指标[左室收缩末期内径(left ventricular end diastolic diameter,LVEDD)、左室舒张末期内径(left ventricular end systolic diameter,LVESD)、左心室射血分数(left ventricular ejection fraction,LVEF)]、心肌指标[肌钙蛋白I(cardiac troponin I,cTnI)、N端脑钠肽前体(n-terminal pro-brain natriuretic peptide,NT-proBNP)]及不良反应发生率。结果两组治疗前LVEDD、LVEF、LVESD、NT-proBNP、cTnI水平比较,差异无统计学意义(P>0.05);治疗后,观察组治疗有效率为97.06%高于对照组,差异有统计学意义(χ^(2)=5.100,P<0.05);与对照组相比,观察组LVEDD、LVESD、NT-proBNP、cTnI水平更低,LVEF更高,差异有统计学意义(P<0.05);两组肌痛、皮肤瘙痒、消化不良、头痛不良反应发生率比较,差异无统计学意(P>0.05)。结论老年冠心病患者应用螺内酯联合瑞舒伐他汀钙治疗可有效控制症状,改善心功能,减轻心肌损伤,且不会增加用药风险。Objective To analyze the efficacy of spironolactone combined with rosuvastatin calcium in the treatment of coronary atherosclerotic heart disease(coronary heart disease)in the elderly.Methods A total of 68 elderly patients with coronary heart disease admitted to Yulin Jiaxian People's Hospital,Shaanxi Province from August 2022 to August 2023 were conveniently selected as the study objects.Thye group were divided into control group and observation group according to random number table method with 34 cases in each group.The control group was treated with rosuvastatin calcium alone,while the observation group was treated with spironolide combined with rosuvastatin calcium.Compared the two groups before and after treatment clinical curative effect and heart function index[left ventricular end systolic diameter(LVEDD),left ventricular end systolic diameter(LVESD),left ventricular ejection fraction(LVEF)],myocardial performance index[cardiac troponin I(cTnI),n-terminal pro-brain natriuretic peptide(NT-proBNP)]and the incidence of adverse reactions.Results There was no statistically significant difference between the levels of LVEDD,LVEF,LVESD,NT-proBNP,and cTnI in the two groups before treatment(P>0.05).After treatment,the effective rate of the observation group was 97.06%higher than that of the control group,and the difference was statistically significant(χ^(2)=5.100,P<0.05).Compared with the control group,the levels of LVEDD,LVESD,NT-proBNP and cTnI in the observation group were lower,while LVEF was higher,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the incidence of myalgia,pruritus,dyspepsia and headache between the two groups(P>0.05).Conclusion The application of spironolactone combined with rosuvastatin calcium treatment in elderly patients with coronary heart disease can effectively control symptoms,improve cardiac function,reduce myocardial injury,and does not increase the risk of medication.
