摘 要:目的探究血常规、呼吸道病原体抗体、C反应蛋白(CRP)联合检测对小儿急性肺炎的诊断价值。方法选取100例呼吸道感染患儿,根据临床初期诊断结果分为A组(细菌感染)和B组(病毒感染),各50例;另选取50例健康体检儿童作为C组。三组均进行血常规、CRP及呼吸道病原体抗体检测。比较三组血常规、CRP水平;比较三组呼吸道病原体抗体检测结果;比较单项检测与联合检测对A组和B组的诊断效能(灵敏度、特异度、准确度)。结果B组白细胞计数(WBC)、CRP水平分别为(6.51±2.85)×10^(9)/L、(5.29±2.96)mg/L,C组WBC、CRP水平分别为(5.94±2.03)×10^(9)/L、(4.31±2.28)mg/L,均低于A组的(15.93±5.96)×10^(9)/L、(40.57±20.11)mg/L,差异有统计学意义(P<0.05);B组WBC、CRP水平与C组比较,差异无统计学意义(P>0.05)。B组呼吸道病原体抗体阳性检出率90.00%显著高于A组的78.00%和C组的0,差异有统计学意义(P<0.05)。以肺部超声检查结果作为金标准,检测出细菌感染患儿45例,病毒感染患儿48例。A组联合检测的灵敏度97.78%(44/45)、特异度100.00%(5/5)、准确度98.00%(49/50)均高于WBC的77.78%(35/45)、40.00%(2/5)、74.00%(37/50),CRP的82.22%(37/45)、40.00%(2/5)、78.00%(39/50),呼吸道病原体抗体的80.00%(36/45)、40.00%(2/5)、76.00%(38/50)。B组联合检测的灵敏度97.92%(47/48),特异度100.00%(2/2),准确度98.00%(49/50),均高于WBC的72.92%(35/48)、50.00%(1/2)、72.00%(36/50),CRP的79.17%(38/48)、50.00%(1/2)、78.00%(39/50),呼吸道病原体抗体的91.67%(44/48)、50.00%(1/2)、90.00%(45/50)。结论血常规中WBC、呼吸道病原体抗体联合CRP检测具有较高的灵敏度、特异度、准确度,可应用于临床上小儿急性肺炎的诊断,有效提高临床诊断率,帮助医生为患儿制定科学有效的治疗方案以缓解患儿的症状,保障患儿的人身安全。Objective To investigate the diagnostic value of combined detection of blood routine,respiratory tract pathogen antibody and C-reactive protein(CRP)in children with acute pneumonia.Methods A total of 100 children with respiratory tract infection were selected and divided into group A(bacterial infection)and group B(viral infection)according to the initial clinical diagnosis results,with 50 cases in each group.Another 50 healthy children were selected as group C.Blood routine,CRP levels and respiratory tract pathogen antibody were detected in all three groups.Blood routine and CRP levels were compared among the three groups.The results of detection of respiratory tract pathogen antibody were compared among the three groups.The diagnostic efficacy(sensitivity,specificity,accuracy)of single detection and combined detection were compared between group A and group B.Results The white blood cell count(WBC)and CRP levels in group B were(6.51±2.85)×10^(9)/L and(5.29±2.96)mg/L,and those in group C were(5.94±2.03)×10^(9)/L and(4.31±2.28)mg/L,which were lower than those of(15.93±5.96)×10^(9)/L and(40.57±20.11)mg/L in group A,and the differences were statistically significant(P<0.05).There was no statistically significant difference in WBC and CRP levels between group B and group C(P>0.05).The positive detection rate of respiratory tract pathogen antibody in group B was 90.00%,which was significantly higher than that of 78.00%in group A and 0 in group C,and the difference was statistically significant(P<0.05).Using the results of pulmonary ultrasound as the gold standard,45 cases of bacterial infection and 48 cases of viral infection were detected.In group A,the sensitivity of the combined detection was 97.78%(44/45),the specificity was 100.00%(5/5)and the accuracy was 98.00%(49/50),which were higher than those of 77.78%(35/45),40.00%(2/5)and 74.00%(37/50)of WBC detection,and 82.22%(37/45),40.00%(2/5)and 78.00%(39/50)of CRP detection,and 80.00%(36/45),40.00%(2/5),76.00%(38/50)of respiratory tract pathogen antibody d
