贵州省康复医疗资源现况及医疗服务质量与安全分析

摘　　要：目的对贵州省康复医疗资源现况进行调查,监测分析贵州省哨点医院康复质控月度数据,综合分析评价我省目前康复医疗服务质量与安全水平。方法对2021年贵州省康复医疗资源调查结果和2023年1-9月贵州省17家国家级康复哨点医院月度质控数据进行分析。结果2021年全省康复医疗资源调查显示,全省108家医疗机构康复医学科平均床位数比6.28%,科室平均总面积731.79 m^(2),仅19.79%的医疗机构康复医学科科主任为康复专业执业范围,13.31%的康复医师为康复执业范围。各级医院设置的亚专业康复治疗室比例最低的为康复工程室(18.18%)。全省平均康复治疗有效率91.92%,住院患者平均康复功能评定率93.68%,平均住院日为21.25 d。全省康复医学科主要收治病种排序依次为神经系统疾病(87.0%)、骨与关节疾病(85.2%)、慢性疼痛(83.3%)、脊髓损伤(81.5%)、儿童疾病(63.9%)、内脏疾病(39.8%)。2023年1-9月,收集17家国家级康复哨点医院上报的质控月度数据共150份。其中床位比中位数稳定在3.3%左右,医师、治疗师和护士床配比中位数分别维持在20%、25%、25%。脑卒中患者早期康复介入率中位数基本维持在40%左右,脊髓损伤和髋、膝关节置换术后患者早期康复介入率在9个月期间波动较大。出院患者日常生活活动能力ADL改善率稳定在95%左右。脑卒中患者运动功能评定率和肩痛预防实施率稳定在100%,其他各项指标均有波动:脑卒中患者言语功能评定率42.86%~57.89%、脑卒中患者吞咽功能评定率32.26%~55.56%、脑卒中后肩痛发生率4.56%~11.70%,脊髓损伤患者泌尿系感染发生率0%~7.14%,住院患者静脉血栓栓塞症发生率0%~0.31%。结论贵州省康复专业人员仍存在较大缺口。贵州省康复治疗亚专业开展不均衡,在言语吞咽治疗和康复工程方面尤为显著,与我省康复医学科主要收治第一病种为神经系统疾病的现况极不�Objective The study aims to investigate the current status of rehabilitation medical resources in Guizhou Province,monitor and analyze the monthly quality control data of designated hospitals in Guizhou Province,and comprehensively evaluate the quality and safety levels of rehabilitation medical services in the province.Methods The study analyzed the results of a 2021 survey on rehabilitation medical resources in Guizhou Province and monthly quality control data from January to September 2023 of 17 national rehabilitation sentinel hospitals in the province.Results The 2021 provincial survey revealed that the average number of beds in rehabilitation departments across 108 medical institutions was 6.28%of total beds,with an average departmental area of 731.79 square meters.Only 19.79%of the department heads in rehabilitation medicine were specialized in rehabilitation,and just 13.31%of rehabilitation physicians had rehabilitation qualifications.The lowest proportion of sub-specialty rehabilitation treatment rooms was in rehabilitation engineering(18.18%).The average effectiveness rate of rehabilitation treatment across the province was 91.92%,with an average rehabilitation function assessment rate of 93.68%for inpatients,and the average length of hospital stay was 21.25 days.The primary diseases treated in rehabilitation departments,in order of frequency,were neurological disorders(87.0%),bone and joint diseases(85.2%),chronic pain(83.3%),spinal cord injuries(81.5%),pediatric diseases(63.9%),and visceral diseases(39.8%).From January to September 2023,150 sets of monthly quality control data were collected from 17 national rehabilitation sentinel hospitals.The median bed ratio was stable at around 3.3%,and the median bed ratios for physicians,therapists,and nurses were around 20%,25%,and 25%,respectively.The median rate of early rehabilitation intervention for stroke patients was roughly 40%,with significant fluctuations over the nine months for early rehabilitation interventions for spinal cord injuries and postope
