不同剂量甲泼尼龙佐治儿童难治性肺炎支原体肺炎临床疗效观察

摘　　要：目的 观察不同剂量甲泼尼龙辅助治疗儿童难治性肺炎支原体肺炎(RMPP)的临床价值,用以探究甲泼尼龙最适宜剂量。方法 选择2019年2月至2022年8月我院126例RMPP患儿。入院后均给予相关治疗,并加用甲泼尼龙,根据治疗剂量将患儿分为大剂量组[4 mg/(kg·d)]、中剂量组[3 mg/(kg·d)]、小剂量组[2 mg/(kg·d)],各42例。比较3组治疗效果、临床表现缓解时间、药物不良反应发生率。结果 大剂量组总有效率(92.86%)高于小剂量组(64.29%)、中剂量组(71.43%),差异显著(P <0.05);大剂量组热退时间、肺啰音消退时间、咳嗽消退时间、住院时间较小剂量组、中剂量组短(P <0.05);3组药物不良反应发生率对比,差异无统计学意义(P> 0.05)。结论 采用[4 mg/(kg·d)]甲泼尼龙佐治儿童RMPP效果较其他剂量更好,能迅速缓解临床症状,且安全性良好。Objective Analyze the clinical efficacy of different doses of methylprednisolone in the treatment of refractory Mycoplasma pneumoniae pneumonia(RMPP)in children,in order to explore the optimal therapeutic dose of methylprednisolone.Methods Selected 126 children with RMPP children from February 2019 to August 2022.After admission,relevant treatment is given,and methylprednisolone is added.According to the treatment dose,the children were divided into large-dose group[4 mg/(kg·d)],medium dose group[3 mg/(kg·d)],and small-dose group[2 mg/(kg·d)],with 42 cases in each group.Compare the treatment effect of the treatment,the time of the clinical manifestations,and the incidence of adverse drug reactions.Results The total effective rate of large-doses group(92.86%)was higher than that of small-doses group(64.29%),medium dose group(71.43%),and significant differences(P<0.05).The large-dose group had shorter fever resolution time,lung rale resolution time,cough resolution time,hospitalization time in the small-dose group and medium dose group(P<0.05).The incidence of adverse reactions in the three groups of drugs is compared,and the difference is not statistically significant(P>0.05).Conclusions The use of[4 mg/(kg·d)]methylprednisolone for the treatment of RMPP in children is more effective than other doses,can quickly alleviate clinical symptoms,and has good safety.
