炙甘草汤加减治疗室性期前收缩的临床疗效观察

摘　　要：目的 观察炙甘草汤加减治疗室性期前收缩(PVCs)的临床疗效。方法 选取北京市昌平区沙河医院2020年5月至2022年5月收治的78例PVCs患者为研究对象,随机分为对照组、治疗组各39例。对照组采用胺碘酮治疗,治疗组采用炙甘草汤加减治疗,每日1剂水煎300 ml,早晚各150 ml分服,治疗周期为1个月。比较两组患者治疗前、治疗1个月后中医证候积分、PVCs次数、PVCs指数、左心室射血分数(LVEF)、每搏输出量(SV)、左心室收缩末期内径(LVESD)变化情况,以及两组的有效性及安全性。结果 治疗1个月后,两组中医证候积分均较治疗前降低,PVCs次数均较治疗前下降,PVCs指数均较治疗前升高,LVEF、SV均较治疗前升高,LVESD较治疗前下降,且治疗组均优于对照组(均P<0.05)。治疗组、对照组的治疗有效率分别为94.87%、82.65%,治疗组高于对照组(P=0.004)。治疗组、对照组不良反应发生率分别为5.13%、20.51%,治疗组低于对照组(P=0.029)。结论 与胺碘酮相比,炙甘草汤加减治疗PVCs疗效确切,不良反应低,可改善心功能。Objective To observe the clinical effect of Zhigancao decoction in the treatment of premature ventricular contractions(PVCs).Methods A total of 78 patients with PVCs admitted to Beijing Changping District Shahe Hospital from May 2020 to May 2022 were randomly divided into control group and treatment group,with 39 cases in each group.The control group was treated with amiodarone,and the treatment group was treated with Zhigancao Decoction.One dose of water was decocted 300 ml per day,150 ml in the morning and 150 ml in the evening.The changes of TCM syndrome score,PVCs frequency,PVCs index,left ventricular function index[left ventricular ejection fraction(LVEF),stroke volume(SV),left ventricular end systolic diameter(LVESD)]were compared between the two groups before and after 1 month of treatment,as well as the effectiveness and safety differences between the two groups.Results After 1 month of treatment,the TCM evidence points of both groups were lower than before treatment,the number of PVCs were lower than before treatment,the index of PVCs were higher than before treatment,the LVEF and SV were higher than before treatment,and the LVESD was lower than before treatment,and all of them were better in the treatment group than in the control group(all P<0.05).The therapeutic efficiency of the treatment group and the control group was 94.87%and 82.65%,respectively,and the treatment group was higher than the control group(P=0.004).The incidence rates of adverse reactions in the treatment group and the control group were 5.13%and 20.51%,respectively,and the treatment group was lower than the control group(P=0.029).Conclusions Compared with amiodarone,Zhigancao decoction has definite curative effect for PVCs,low adverse reactions,and can improve cardiac function.
