ERAS理念下探讨责任制护理对四肢骨折患者疼痛控制、应激反应及压疮的影响

摘　　要：目的 探讨ERAS理念下责任制护理对四肢骨折患者疼痛控制、应激反应及压疮的影响。方法 选取2021年9月至2022年3月福建中医药大学附属第三人民医院收治的60例四肢骨折围手术期患者作为研究对象,采用随机、单盲的方法,分为常规组和研究组,各30例。常规组采用常规护理,研究组在常规组护理方法上进行ERAS理念下责任制护理,观察两组患者护理前后疼痛、应激反应、压疮及并发症。结果 护理前,常规组及研究组患者的VAS评分比较无统计学意义(P>0.05),护理后,两组患者VAS评分均降低(P<0.05),与常规组相比,研究组患者VAS评分更低(P<0.05);护理前,常规组及研究组患者的应激反应指标比较无统计学意义(P>0.05),护理后,两组患者肾上腺素、去甲肾上腺素及肾素水平均降低(P<0.05),与常规组相比,研究组患者肾上腺素、去甲肾上腺素及肾素水平更低(P<0.05);常规组及研究组患者的压疮发生情况、并发症发生率比较无统计学意义(P>0.05)。结论 ERAS理念下探讨责任制护理可以显著抑制四肢骨折患者疼痛,降低应激反应及压疮,改善并发症。Objective To explore the effect of responsibility nursing under the concept of ERAS on pain control,stress response and pressure ulcers in patients with limb fractures.Methods From September 2021 to March 2022,60 patients with limb fractures in our hospital during the perioperative period were selected as the research objects.According to the random and single-blind method,they were divided into a routine group and a research group,with 30 cases in each group.The routine group was given routine nursing,while the research group was given the responsibility system nursing under the ERAS concept based on the nursing method of the routine group.Pain,stress response,pressure ulcers and complications were observed in the two groups before and after nursing.Results Before nursing care,the comparison of VAS scores between patients in the conventional group and the study group was not statistically significant(P>0.05),and after nursing care,the VAS scores of patients in both groups were reduced(P<0.05),and compared with the conventional group,the VAS scores of patients in the study group were lower(P<0.05);before nursing care,the comparison of stress response indexes between patients in the conventional group and the study group was not statistically significant(P>0.05),and after nursing care,the The levels of epinephrine,norepinephrine and renin were lower in both groups(P<0.05),and compared with the conventional group,the levels of epinephrine,norepinephrine and renin were lower in the study group(P<0.05);there was no statistical significance in comparing the incidence of pressure ulcers and the incidence of complications between the conventional group and the study group(P>0.05).Conclusions Discussing responsibility nursing under the concept of ERAS can significantly inhibit pain in patients with limb fractures,reduce stress response and pressure ulcers,and improve complications.
