交叉电项针治疗急性脑缺血性脑卒中后假性球麻痹所致吞咽障碍验案1例

摘　　要：急性缺血性脑卒中所致的假性球麻痹为临床常见的并发症之一,吞咽障碍为其常见的表现,轻症患者进食饮水呛咳,容易误吸,从而导致卒中相关性肺炎,或因进食饮水不足导致电解质紊乱、营养不良等一系列并发症,重症患者需长期留置胃管,降低了患者的生活质量。既往相关针刺及康复治疗研究报道较多,在临床上取得了一些效果,但采用交叉电项针方式联合康复治疗来改善这类患者的吞咽功能的报道为数不多。作者在临床上利用交叉电项针联合康复治疗急性缺血性脑卒中后合并假性球麻痹导致吞咽障碍的1例患者,明显改善了其吞咽功能,使其在短期住院期间顺利拔除胃管、恢复了经口进食水,提高了患者的生活质量,增强了患者的信心。现写为个案报道,以期为今后临床医师治疗这类患者提供临床思路。Pseudobulbar palsy is one of the common clinical complications caused by the acute ischemic stroke,Dysphagia is a common manifestation.Patients with mild symptoms like choke on food and water are prone to accidental inhalation,which leads to strokerelated pneumonia or electrolyte disorder,malnutrition,aggravating the primary disease and a series of complications due to insufficient food and water.Patients with severe symptoms need long-term indwelling gastric tube,which reduces the quality of life of patients.There are many reports on acupuncture and rehabilitation therapy in the past,and some clinical effects have been achieved.However,there are few clinical reports on improving the dysphagia of these patients by means of cross nape electroacupuncture combined with rehabilitation therapy.In this paper,one patient with dysphagia caused by pseudobulbar paralysis after acute ischemic stroke were treated by cross nape electroacupuncture combined with rehabilitation,the swallowing function was significantly improved,the gastric tube was removed smoothly during the short-term hospitalization,and the oral feeding and drinking was restored.The quality of life of the patients was improved,and the self-confidence of the patients was enhanced.It is written as a case report in order to provide clinical ideas for clinicians in the treatment of such patients in the future.
