摘 要:目的探讨12周运动康复对冠状动脉粥样硬化性心脏病(冠心病)患者运动耐力与心功能的影响。方法选择2020年6月至2022年6月于首都医科大学附属北京康复医院就诊的冠心病患者100例进行研究,通过抽签法分为观察组和对照组,每组各50例,对照组采用常规康复训练,观察组采用运动康复训练,两组康复训练均为12周。比较两组患者治疗前后6 min步行距离(6MWD)、峰值通气率(PVR)、峰值摄氧量(VO_(2)max)、舒张早期二尖瓣血流频谱E峰峰值与舒张早期二尖瓣环最大运动E'峰峰值之比(E/E')、舒张早期二尖瓣血流频谱E峰峰值与舒张晚期二尖瓣血流频谱A峰峰值之比(E/A)、左室舒张末期内径(LVEDD)、左室收缩末期内径(LVESD)、左室射血分数(LVEF)的水平变化,并记录治疗期间的不良事件。结果观察组治疗后6MWD、PVR、VO_(2)max、E/E'水平均明显高于对照组,E/A水平明显低于对照组(P<0.05);观察组治疗后的LVEDD、LVESD明显低于对照组,LVEF明显高于对照组(P<0.05);观察组不良事件总发生率明显低于对照组,差异有统计学意义(P<0.05)。结论12周运动康复能提高冠心病患者的运动耐力并改善心功能。Objective To discuss the influence of 12-week exercise rehabilitation on exercise endurance and heart function in patients with coronary heart disease(CHD).Methods CHD patients(n=100)were chosen from Beijing Rehabilitation Hospital affiliated to Capital Medical University from June 2020 to June 2022,and divided randomly into observation group and control group(each n=50).The control group was given routine rehabilitation training,and observation group was given exercise rehabilitation training for 12 weeks.The changes of 6-minute walk distance(6MWD),peak ventilation rate(PVR),peak oxygen consumption(VO_(2)peak),the ratio of peak value of mitral valve flow spectrum E in early diastole to peak value of maximum mitral ring movement E'in early diastole(E/E'),the ratio of peak value of mitral valve flow spectrum E in early diastole to peak value of mitral valve flow spectrum A in late diastole(E/A),left ventricular end-diastolic diameter(LVEDD),left ventricular end-systolic diameter(LVESD)and left ventricular ejection fraction(LVEF)were compared between 2 groups before and after training.The adverse events were recorded during training period.Results The levels of 6MWD,PVR,VO_(2)peak and E/E'were significantly higher,and E/A level was significantly lower in observation group than those in control group after training(P<0.05).The levels of LVEDD and LVESD were significantly lower,and LVEF level was significantly higher in observation group than those in control group after training(P<0.05).The incidence rates of adverse events were significantly lower in observation group than those in control group(P<0.05).Conclusion The exercise rehabilitation for 12 weeks can improve exercise endurance and heart function in CHD patients.
