间断性使用艾考糊精透析液在连续性非卧床腹膜透析患者中的应用

摘　　要：目的评估间断性使用艾考糊精透析液(ICO)对连续性非卧床腹膜透析(CAPD)患者腹膜透析(PD)超滤的影响,为临床间断性使用ICO提供依据。方法回顾性分析本院腹膜透析中心2022年3至8月期间2次/周间断性使用ICO的CAPD患者临床资料,比较患者使用ICO前和使用3个月后PD超滤情况,根据患者腹膜功能将其分为腹膜高转运(包括高转运和高平均转运)和腹膜低转运(包括低平均转运和低转运)两组,分别比较两组患者使用ICO前后的PD超滤量。结果共有29例CAPD患者入选,其中男15例,女14例,平均年龄(54.1±10.2)岁;根据腹膜平衡试验分组,其中腹膜高转运组19例,腹膜低转运组10例,两组患者年龄、性别、透析龄等方面差异无统计学意义(P>0.05);所有患者使用ICO后每日PD净超滤量较使用前明显增加[(641.4±251.7)VS.(478.3±311.4),P=0.032];腹膜高转运组使用ICO后每日PD净超滤量也明显增加[(656.8±247.8)VS.(417.6±310.2),P=0.013],而腹膜低转运组使用ICO后每日PD净超滤量无明显变化,差异无统计学意义(P>0.05)。结论每周2次间断性使用ICO可显著提高CAPD患者PD超滤量,尤其对于腹膜高转运和高平均转运患者效果更明显。Objective To investigate the impact of intermittent Icodextrin(ICO)use on the ultrafiltration volume in Continuous non bedridden peritoneal dialysis(CAPD)patients.Methods We recruited CAPD patients using ICO twice a week who treated in Peritoneal Dialysis Center of Hangzhou Hospital of TCM from March to August in 2022,and evaluate the difference of peritoneal ultrafiltration after using ICO.According to D/Pcr and Twardowski standard,all the patients were divided into the High group(including high transport and high-average transport)and Low group(including low transport and low-average transport),and the difference of peritoneal ultrafiltration between two groups were evaluated.Results The study enrolled 29 CAPD patients,comprising 15 males and 14 females,with an average age of(54.1±10.2),and 19 were in the High group,and 10 in the Low group.The ultrafiltration of all the 29 patients was much higher than before after using ICO[(641.4±251.7)VS.(478.3±311.4),P=0.032].There was no significant difference between the two groups in age,gender,and time of dialysis.The ultrafiltration of the High group was also much higher after using ICO[(656.8±247.8)VS.(417.6±310.2),P=0.013].By contrast,the increased volume was not significantly difference in the Low group(P>0.05).Conclusion Twice a week using of ICO can improve the ultrafiltration for the CAPD patients,especially for the high transport and high-average transport groups.
