“鸡尾酒”式混合液在老年股骨粗隆间骨折术后止痛疗效及早期功能复健中的作用

摘　　要：目的探讨“鸡尾酒”式混合液在老年股骨粗隆间骨折术后止痛效果评估及早期功能复健中的作用。方法选择2018年1月至2021年6月收治的老年股骨粗隆间骨折手术患者80例,采用随机双盲分为观察组及对照组,观察组给予常规镇痛药物处置外术区切口周围软组织注射鸡尾酒式混合液,对照组除常规镇痛外未进行特殊处理,比较两组术后静息及运动状态下6 h、12 h、24 h、48 h、72 h VAS疼痛评分,采用匹兹堡睡眠质量指数(PSQI)评价两组患者术后睡眠质量,采用Harris髋关节功能评分对两组患者术后第3天及第7天髋关节功能进行评价。结果应用VAS评分,评估两组患者在术后静息状态及运动状态下6 h、12 h、24 h、48 h、72 h分值,观察组低于对照组,差异有统计学意义(P<0.05);采用PSQI评价两组患者术后睡眠情况,观察组较对照组有明显的改善,差异有统计学意义(P<0.05);应用Harris评分,评价两组患者在术后第3天及第7天髋关节功能,观察组优于对照组,差异有统计学意义(P<0.05)。结论在老年股骨粗隆间骨折患者术区切口周围软组织注射鸡尾酒式镇痛混合液在患者术后疼痛改善起到积极作用,可提高患者术后睡眠质量,在早期髋关节功能复健中发挥重要作用,且操作方法简便,疗效确切,值得临床应用。Objective To explore the effect of"cocktail"therapy on postoperative pain and early rehabilitation of femoral intertrochanteric fracture in the elderly.Methods 80 elderly patients who were admitted into our hospital with femoral intertrochanteric fracture from January 2018 to June 2021 were enrolled in this study.After admission,they were randomly divided into the observation group and the control group in a double-blind way.In addition to conventional analgesia,the observation group was injected with a“cocktail”analgesia compound solution,and the control group received no special treatment except routine analgesia.Resting and active VAS scores at 6 h,12 h,24 h,48 h and 72 h after the operation were observed and compared between the two groups.The Pittsburgh Sleep Quality Index(PSQI)was used to evaluate the postoperative sleep quality of the two groups,and the Harris score was used to evaluate the hip function on day 3 and day 7 after the surgery.Results Resting and active VAS scores of the observation group at 6 h,12 h,24 h,48 h and 72 h were lower than those of the control group,and the difference was statistically significant.The sleep quality of the observation group was significantly improved compared with the control group.The hip function scores of the observation group were better than those of the control group at 3 and 7 days after surgery,and the differences were statistically significant(P<0.05).Conclusion Injecting cocktail analgesic mixture into soft tissue around the incision of the elderly patients with intertrochanteric fracture of femur plays an active role in improving postoperative pain,improving postoperative sleep quality and playing an important role in early rehabilitation of hip joint function.The operation method is simple,the curative effect is exact,and it is worthy of application.
