补肾通络方联合氨基葡萄糖胶囊治疗膝关节骨性关节炎的疗效及对NO、SOD、CRP的影响

摘　　要：目的:研究补肾通络方联合氨基葡萄糖胶囊治疗膝关节骨性关节炎的临床疗效及对患者血清一氧化氮(nitric oxide,NO)、超氧化物歧化酶(superoxide dismutase,SOD)及超敏C反应蛋白(high-sensitivity C-reactive protein,hs-CRP)水平的影响。方法:将膝关节骨性关节炎患者120例按照随机数字表法分为对照组和观察组各60例。对照组给予氨基葡萄糖胶囊治疗,观察组在对照组基础上给予补肾通络方治疗,疗程为6个月,比较两组患者治疗前后临床疗效、中医证候积分,视觉模拟评分(visual analog scale,VAS)、西安大略和麦克马斯特大学(Western Ontario and McMaster Universities,WOMAC)骨关节炎指数评分、膝关节功能Lysholm评分及血清NO、SOD、hs-CRP水平与安全性。结果:观察组总有效率为93.33%(56/60),高于对照组的70.00%(42/60)(P<0.05);治疗后两组患者中医证候积分均较治疗前降低(P<0.05),观察组低于对照组(P<0.05);治疗后两组患者膝关节VAS、WOMAC、Lysholm评分改善均优于治疗前(P<0.05),观察组优于对照组(P<0.05);治疗后两组患者血清NO及hs-CRP水平均较治疗前下降,SOD水平均较治疗前升高,观察组NO、hs-CRP水平低于对照组(P<0.05),SOD水平高于对照组(P<0.05);治疗过程中两组患者均无明显不良反应发生。结论:补肾通络方联合氨基葡萄糖胶囊可提高膝关节骨性关节炎患者临床疗效,抑制血清NO及hs-CRP水平,促进血清SOD表达,有效恢复患者关节功能,效果优于单用氨基葡萄糖胶囊。Objective:To study clinical effects of Bushen Tongluo prescription and glucosamine capsules in treating knee osteoarthritis(KOA)and its influence on the levels of NO,SOD and hs-CRP.Methods:All 120 KOA patients were allocated to the control group and the observation group according to random number table method with 60 cases in each group.The control group was treated by glucosamine capsules,and the observation group by Bushen Tongluo prescription based on the therapy the control group accepted.The course was six months.To compare clinical effects,TCM syndrome integrals,VAS,WOMAC,Lysholm,the levels of NO,SOD and hs-CRP,and the safety between both groups before and after the treatment.Results:Total effective rate of the observation group was 93.33%(56/60),higher than 70.00%(42/60)of the control group(P<0.05);after the treatment,TCM syndrome integrals were lowered than these before the treatment in both groups(P<0.05),the observation group was lower than the control group(P<0.05).After the treatment,VAS,WOMAC and Lysholm scores of both groups were better than these before the treatment(P<0.05),the observation group was better than the control group(P<0.05).After the treatment,the levels of NO and hs-CRP were lowered in both groups compared with these before the treatment,the levels of SOD were increased than these before the treatment,the levels of NO and hs-CRP of the observation group were lower than these of the control group(P<0.05),the levels of SOD were higher than these of the control group(P<0.05).NO obvious adverse reaction happened during therapeutic process in both groups.Conclusion:Bushen Tongluo prescription and glucosamine capsules could improve clinical effects of KOA patients,inhibit the levels of NO and hs-CRP,promote the expressions of SOD and effectively restore the patients'joint function and its effects better than these of single use of the capsules.
