克氏针内固定结合外固定支架与锁定钢板内固定治疗桡骨远端粉碎性骨折的效果比较

摘　　要：目的 比较克氏针内固定结合外固定支架与锁定钢板内固定治疗桡骨远端粉碎性骨折的临床效果。方法 选取2020年8月至2022年7月抚州市第二人民医院骨科收治的60例桡骨远端粉碎性骨折患者为研究对象,采用随机数字表法分为对照组和研究组,各30例。对照组采用锁定钢板内固定治疗,研究组采用克氏针内固定结合外固定支架治疗,对所有患者术后随访3个月。比较两组患者围手术期指标、影像学指标、腕关节功能优良率及并发症发生率。结果 研究组的手术时间及住院时间均短于对照组,住院费用低于对照组,差异有统计学意义(P<0.05)。研究组术后的掌倾角、尺偏角及桡骨高度大于对照组,差异有统计学意义(P<0.05)。研究组腕关节功能优良率为90.00%,高于对照组的66.67%,差异有统计学意义(P<0.05)。研究组并发症总发生率低于对照组,差异有统计学意义(P<0.05)。结论 桡骨远端粉碎性骨折患者在克氏针内固定与外固定支架联合治疗下获得理想的效果,有助于促进腕关节功能的改善,同时还可改善影像学各指标水平,安全可靠,值得推广应用。Objective To investigate the clinical effect of Kirschner wire internal fixation combined with external fixator and locking plate internal fixation in the treatment of comminuted distal radius fractures.Methods A total of 60 patients with comminuted distal radius fracture admitted to the Department of Orthopedics of Second People's Hospital of Fuzhou City from August 2020 to July 2022 were selected as the study objects,and they were divided into control group and study group by random number table method,with 30 cases in each group.The control group was treated with locking steel plate internal fixation,while the study group was treated with Kirschner wire internal fixation combined with external fixation brackets.All patients were followed up for 3 months after surgery.Perioperative indexes,imaging indexes,excellent and good rate of wrist joint function and complication rate were compared between the two groups.Results The operation time and hospitalization time of the study group were shorter than that of the control group,and the hospitalization cost was lower,with statistical significances(P<0.05).The palmar inclination angle,ulnar declination angle and radius height of the study group were higher than those of the control group,and the differences were statistically significant(P<0.05).The excellent and good rate of wrist joint function in the study group was 90.00%,which was higher than 66.67% in the control group,and the difference was statistically significant(P<0.05).The total complication rate of the study group was lower than that of the control group,and the difference was statistically significant(P<0.05).Conclusion Patients with comminuted distal radius fractures can achieve ideal results under the combined treatment of Kirschner wire internal fixation and external fixation bracket,which helps to promote the improvement of wrist joint function and also improves the level of imaging indicators.It is safe and reliable,and is worth promoting and applying.
