儿科药师参与多学科会诊的效果分析

摘　　要：目的通过分析我院临床药师参与儿科多学科会诊情况,及参与会诊对临床合理用药及临床考核指标的促进作用情况,探讨儿科临床药师参与临床制定个体化治疗方案及会诊模式的效果。方法回顾分析2021年1至12月儿科临床药师参与的213例多学科会诊案例,分析会诊数量排名前5名的科室1年来的合理用药指标,并与2020年合理用药指标进行比较。结果在213例病例中,会诊意见全部被临床采纳的为208例(97.65%),部分采纳的为1例(0.47%),未采纳的为4例(1.88%)。213例病例中,患儿痊愈95例(44.60%);好转108例(50.70%);放弃治疗2例(0.94%);转上级医院治疗8例(3.76%)。2021年会诊科室合理用药指标较2020年明显提升,随着多学科药学会诊模式进一步完善,临床药师会诊能力大幅度提升。结论随着临床药师深入临床工作,临床医师对药师的需求增加。临床药师通过参与多学科会诊能够提升临床合理用药水平,同时临床考核指标得到大幅度改善。AIM To explore the engagement of pediatric clinical pharmacists in the clinical development of individualized treatment plans and consultation modes through the analyze of the participation of clinical pharmacists in pediatric multidisciplinary consultations in our hospital,and the promotional effect of their involvement in multidisciplinary consultations on clinical rational drug use and clinical assessment indexes.METHODS Reviewed and analyzed 213 cases of multidisciplinary consultations involving pharmacists from January to December 2021.Examined the indexes of rational drug use in the top 5 departments based on the number of consultations over the past year and compared them with the rational drug use indexes from 2020.RESULT Among 213 cases,the opinions provided during consultations were fully adopted in 208 cases(97.65%),partially adopted in 1 case(0.47%),and not adopted in 4 cases(1.88%).Among the 213 cases,95 cases(44.60%)achieved recovery,108 cases(50.70%)improved,2 cases discontinued treatment(0.94%)and 8 cases(3.76%)were transferred to a higher-level hospital.The indexes of rational drug use in consultation departments in 2021 showed a significant improvement compared with that in 2020.The multidisciplinary pharmaceutical consultation model was further refined,and the clinical pharmacists'consultation capabilities had a substantial increase.CONCLUSION As clinical pharmacists become more involved in clinical practic,there is an increasing demand for clinical pharmacists.Pharmacists through their participation in multidisciplinary consultations can enhance their level of rational drug use,and the clinical assessment indexes can be greatly improved.
