摘 要:目的探究血清同型半胱氨酸(Hcy)及亚甲基四氢叶酸还原酶(MTHFR)基因C677T多态性与不良妊娠史的相关性。方法选取2019年1月至2021年6月佛山复星禅诚医院收治的257例存在不良妊娠史女性作为病例组,另选取同期186名正常妊娠女性作为对照组。两组均行MTHFR C677T基因多态性及血清Hcy检测,比较两组MTHFR C677T基因型、血清Hcy水平及不同MTHFR C677T基因型女性Hcy水平,分析血清Hcy水平和MTHFR C677T基因型与不良妊娠的相关性。结果两组MTHFR C677T基因型频率、等位基因频率比较差异有统计学意义(P<0.05);基因型分布CC型最高,CT型次之,TT型最低。病例组血清Hcy水平高于对照组,差异有统计学意义(P<0.05)。MTHFR C677基因型TT的Hcy水平高于基因型CC型和CT型(P<0.05);MTHFR C677基因型CC型与CT型Hcy水平比较差异无统计学意义。二元Logistic回归分析结果显示,CT型发生不良妊娠的风险为参照的2.134倍(P<0.05),TT型发生不良妊娠的风险为参照的2.585倍(P<0.05);CT型、TT型、血清Hcy水平是不良妊娠的危险因素(P<0.05)。血清Hcy水平每升高1个单位时,不良妊娠的发生风险增加1.149倍(P<0.05)。结论妊娠女性MTHFR C677T基因多态性及血清Hcy浓度与不良妊娠相关,MTHFR C677T突变基因型和血清Hcy水平升高是女性不良妊娠的危险因素。Objective To study the correlation between serum homocysteine(Hcy)and methylenetetrahydrofolate reductase(MTHFR)gene C677T polymorphism and adverse pregnancy history.Methods A total of 257 women with adverse pregnancy history admitted to Foshan Fosun Chancheng Hospital from January 2019 to June 2021 were selected as the case group,and 186 normal pregnant women in the same period were selected as the control group.MTHFR C677T gene polymorphism and serum Hcy were detected in the two groups,the MTHFR C677T genotype,serum Hcy level and Hcy level of women with different MTHFR C677T genotypes were compared between the two groups,the correlation between serum Hcy level,MTHFR C677T genotype and adverse pregnancy was analyzed.Results There were significant differences in MTHFR C677T genotype frequency and allele frequency between the two groups(P<0.05).The genotype distribution of CC type was the highest,followed by CT type and TT type was the lowest.The serum Hcy level in the case group was higher than that in the control group,and the difference was statistically significant(P<0.05).The Hcy level of THFR C677 genotype TT was higher than that of genotype CC and CT(P<0.05).There was no significant difference in Hcy level between MTHFR C677 CC genotype and CT genotype.The results of binary logistic regression analysis showed that the risk of adverse pregnancy in CT type was 2.134 times higher than that in reference(P<0.05),and the risk of adverse pregnancy in TT type was 2.585 times higher than that in reference(P<0.05).CT type,TT type and serum Hcy level were risk factors for adverse pregnancy(P<0.05).The risk of adverse pregnancy increased by 1.149 times when the serum Hcy level increased by 1 unit(P<0.05).Conclusion MTHFR C677T gene polymorphism and serum Hcy level in pregnant women are associated with adverse pregnancy,MTHFR C677T mutation genotype and elevated serum Hcy levels are risk factors for adverse pregnancy in women.
