摘 要:目的探讨尿激酶联合地塞米松行胸腔注射给药治疗结核性包裹性胸腔积液的临床疗效。方法选取2020年7月至2021年7月百色市人民医院收治的80例结核性包裹性胸腔积液患者作为研究对象,随机分为对照组、尿激酶组、地塞米松组、联合组,每组20例。对照组给予异烟肼+利福平+乙胺丁醇+吡嗪酰胺的四联抗结核药物治疗,尿激酶组在对照组基础上加用尿激酶胸腔注射治疗,地塞米松组在对照组基础上加用地塞米松胸腔注射治疗,联合组在对照组基础上加用尿激酶联合地塞米松胸腔注射治疗。比较4组临床疗效、住院时间、胸腔积液吸收时间及总量。结果联合用药组治疗总有效率高于对照组,差异有统计学意义(P<0.05);其他各组间总有效率两两比较差异无统计学意义。尿激酶组、地塞米松组、联合用药组住院时间均短于对照组,联合用药组住院时间均短于尿激酶组、地塞米松组,差异有统计学意义(P<0.05);尿激酶组与地塞米松组住院时间比较差异无统计学意义。尿激酶组、地塞米松组、联合用药组胸腔积液吸收时间短于对照组,胸腔积液总量少于对照组,且联合用药组胸腔积液吸收时间短于尿激酶组、地塞米松组,胸腔积液总量少于尿激酶组、地塞米松组,差异有统计学意义(P<0.05);尿激酶组与地塞米松组胸腔积液吸收时间、总量比较差异无统计学意义。结论尿激酶联合地塞米松胸腔注射给药治疗结核性包裹性胸腔积液效果显著,可缩短胸腔积液吸收时间和住院时间,有效减少胸腔积液,值得临床推广应用。Objective To investigate the clinical efficacy of urokinase combined with dexamethasone in the treatment of tuberculous encapsulated pleural effusion.Methods A total of 80 patients with tuberculous encapsulated pleural effusion admitted to People's Hospital of Baise from July 2020 to July 2021 were selected as the research subjects,they were randomly divided into the control group,urokinase group,dexamethasone group and the combined group,with 20 cases in each group.The control group was treated with isoniazid+rifampicin+ethambutol+pyrazinamide quadruple anti-tuberculosis drugs,the urokinase group was treated with urokinase intrapleural injection on the basis of the control group,the dexamethasone group was treated with dexamethasone intrapleural injection on the basis of the control group,and the combined group was treated with urokinase combined with dexamethasone intrapleural injection on the basis of the control group.The clinical effect,hospitalization time,pleural effusion absorption time and total amount were compared among the four groups.Results The total effective rate in the combined group was higher than that in the control group,and the difference was statistically significant(P<0.05);there were no significant differences in the total effective rate between the other groups.The hospitalization time in the urokinase group,dexamethasone group and the combined group was shorter than that in the control group,and the hospitalization time in combined group was shorter than that in the urokinase group and the dexamethasone group,the differences were statistically significant(P<0.05),there was no significant difference in hospitalization time between the urokinase group and the dexamethasone group.The absorption time of pleural effusion in the urokinase group,dexamethasone group and the combined group was shorter than that in the control group,and the total amount of pleural effusion was less than that in the control group,and the absorption time of pleural effusion in the combined group was shorter than th
