摘 要:目的探讨胎儿颈项透明层厚度(NT)超声检测联合孕中期四维彩超筛查在胎儿结构畸形中的应用价值。方法选取2020年7月至2021年5月于江西中医药大学第二附属医院进行产检的100例孕产妇作为研究对象,所有孕妇均行产前二维彩超、NT超声联合孕中期四维彩超检查。以产后结局为金标准,分析NT超声检查与四维彩超检查结果,比较NT超声、四维彩超及联合检测诊断效能。结果孕龄为11~14周时,100例产检产妇中,随访确诊结果异常胎儿17例(17.00%),NT超声检出结构异常胎儿14例(14.00%),检出结构异常前三位为NT增厚(41.18%)、淋巴管囊瘤(23.53%)、脐膨出(17.65%)。在孕龄20~26周时,100例产检孕妇中,随访结构异常胎儿17例(17.00%),四维彩超检出16例(16.00%),检出结构异常类型前3位为泌尿系统异常(52.94%)、颅内结构异常(17.65%)、四肢短小(11.76%)。随访结果显示,共有畸形胎儿17例,正常胎儿83例。Kappa一致性联合检测>四维彩超>NT超声;NT超声、四维彩超及联合检测的灵敏度、特异度、准确度比较差异无统计学意义。结论NT超声检测联合孕中期四维彩超在诊断胎儿结构畸形中临床效果良好,其诊断准确率较高,可一定程度帮助孕妇避免不良妊娠结局。Objective To explore the application value of fetal nuchal translucecy(NT)ultrasound combined with four-dimensional color ultrasound screening in the second trimester in fetal structural malformations.Methods A total of 100 pregnant women who underwent prenatal examination in the Second Affiliated Hospital of Jiangxi University of Traditional Chinese Medicine from July 2020 to May 2021 were selected as the research subjects.All pregnant women underwent prenatal two-dimensional color Doppler ultrasound,NT ultrasound combined with four-dimensional color Doppler ultrasound in the second trimester.Taking the postpartum outcome as the gold standard,the results of NT ultrasonography and four-dimensional color Doppler ultrasonography were analyzed,and the diagnostic efficacy of NT ultrasonography,four-dimensional color Doppler ultrasonography and combined detection was compared.Results The gestational age was 11-14 weeks,among the 100 pregnant women,17 cases(17.00%)were diagnosed as abnormal fetuses by follow-up,and 14 cases(14.00%)were detected as structural abnormalities by NT ultrasound,the top three structural abnormalities were NT thickening(41.18%),lymphangiocystic tumor(23.53%)and omphalocele(17.65%).At the gestational age of 20-26 weeks,17 cases(17.00%)of structural abnormalities were followed up in 100 pregnant women,and 16 cases(16.00%)were detected by four-dimensional color Doppler ultrasound,the top three types of structural abnormalities were urinary system abnormalities(52.94%),intracranial structural abnormalities(17.65%),and short limbs(11.76%).The follow-up results showed that there were 17 cases of malformed fetuses and 83 cases of normal fetuses.Kappa consistency combined detection>four-dimensional color Doppler ultrasound>NT ultrasound;there were no significant differences in the sensitivity,specificity and accuracy of NT ultrasound,four-dimensional color Doppler ultrasound and combined detection.Conclusion NT ultrasound combined with four-dimensional color ultrasound in the second trimester has good
