艾司奥美拉唑镁肠溶片联合西吡氯铵含片对反流性咽喉炎患者的反流症状指数和胃肠激素指标的影响

摘　　要：目的 分析艾司奥美拉唑镁肠溶片联合西吡氯铵含片对反流性咽喉炎患者的反流症状指数和胃肠激素指标的影响。方法 根据纳入及排除标准选取病例,共纳入102例反流性咽喉炎患者,随机分为试验组(51例)和对照组(51例)。对照组给予艾司奥美拉唑镁肠溶片治疗,晨起口服剂40 mg,1次/d;试验组给予艾司奥美拉唑镁肠溶片+西吡氯铵含片,前者剂量同对照组,后者剂量2 mg,3次/d。在治疗8周后,对比2组临床疗效,反流症状指数评分量表及反流体征评分量表,食管下括约肌压力,胃泌素及胃动素指标,食管反流程度DeMeester计分及食管近、远端pH值。结果 治疗8周后,试验组患者临床总有效率(94%,48/51)高于对照组(80%,41/51)(P<0.05);RSI及RFS较治疗前均有所降低,且试验组低于对照组(P<0.05);试验组患者治疗后的食管下括约肌压力高于对照组(P<0.05);试验组患者治疗后的胃泌素及胃动素指标均高于对照组(P<0.05);试验组食管反流程度计分、食管近端pH值、远端pH值<4的时间百分比均低于对照组(P<0.05)。结论 艾司奥美拉唑镁肠溶片联合西吡氯铵含片对反流性咽喉炎的治疗作用相比单一应用艾司奥美拉唑镁肠溶片的效果更佳,在改善患者的肠胃激素水平方面更加高效,缓解临床症状作用更加明显,值得推广使用。Objective To investigate the effect of reflux symptoms on reflux symptom index and gastrointestinal hormone index in patients with reflux pharyngitis.Methods Cases were selected according to the inclusion and exclusion criteria,and a total of 102 patients with reflux pharyngitis were included,which were randomly divided into trial group(n=51)and control group(n=51).The control group was treated with esomeprazole magnesium enteric tablet,40 mg,1 dose/d;the test group was given esomeprazole magnesium enteric tablet as the control group,the latter dose was 2 mg,3 doses/d.After 8 weeks of treatment,the clinical efficacy of the two groups were compared,reflux symptom index scale and reflux sign scale,lower esophageal sphincter pressure,gastrin and gastrin index,degree of esophageal reflux DeMeester score and near and distal esophageal pH values.Results After 8 weeks of treatment,total clinical response rate of patients in the test group(94%,48/51)was higher than that in the control group(80%,41/51)(P<0.05).Both RSI and RFS were reduced from pretreatment and were lower in the test group than in the control group(P<0.05).The pressure of the lower esophageal sphincter was higher than that in the control group(P<0.05).The gastrin and gastrin indexes were higher than those in the control group(P<0.05).The percentage of time scoring the degree of esophageal reflux,proximal pH and distal pH value<4 were lower than that of the control group(P<0.05).Conclusion The therapeutic effect of esomeprazole magnesium enteric tablets combined with cipyramium chloride lozenge on reflux pharyngitis is better than the single effect of esomeprazole magnesium enteric tablets,which is more efficient in improving the gastrointestinal hormone level of patients and has more obvious effect in alleviating clinical symptoms,which is worthy of promotion and use.
