摘　　要：目的 探讨Beers标准(2019)对老年多重用药患者潜在不适当用药(PIM)的评估价值。方法 收集2021年4月-2023年4月南京市中心医院门诊收治的312例老年多重用药患者的临床资料,分析Beers标准(2019)的评价结果,并将患者分为PIM暴露组和非PIM暴露组;Kappa法分析Beers标准(2019)与中国PIM标准识别PIM的一致性;比较两组患者的临床资料;多因素Logistic回归分析患者PIM的风险因素。结果 312例老年多重用药患者发生1~5种PIM的概率为32.37%(101/312),累计发生PIM共165例次;2种标准识别PIM的一致性系数为0.359;多因素Logistic回归分析结果显示,高龄、用药种类多、临床疾病诊断数多是患者发生PIM的危险因素(P<0.05)。结论 Beers标准(2019)评估老年多重用药患者PIM具有重要意义,可与中国PIM标准联合应用,以促进老年患者安全、合理用药。Objective To explore the value of Beers criteria(2019)in the evaluation of potentially inappropriate medication(PIM)in elderly patients with multiple drug use.Methods The clinical data of 312 elderly patients with multiple drug use treated in Nanjing Central Hospital from April 2021 to April 2023 were collected,and the evaluation results of Beers criteria(2019)were analyzed.The patients were divided into PIM exposure group and non-PIM exposure group.Kappa method was used to analyze the consistency between Beers criteria(2019)and Chinese PIM criteria to identify PIM.The clinical data of the two groups were compared.The influencing factors of PIM in the patients were analyzed by multivariate Logistic regression.Results The probability of developing 1 to 5 kinds of PIM in 312 elderly patients with multiple drug use was 32.37%(101/312),and a total of 165 cases of PIM occurred.The consistency coefficient of the two criteria to identify PIM was 0.359.The results of multivariate Logistic regression analysis showed that advanced age,more kinds of drugs and more clinical diseases were the risk factors of PIM(P<0.05).Conclusion Beers criteria(2019)is of great significance to evaluate PIM in elderly patients with multiple drug use,and can be combined with Chinese PIM criteria to promote safe and rational drug use in elderly patients.
