摘 要:目的:探讨血流限制性运动训练对前交叉韧带重建术后患者股四头肌肌力、足偏角及足底冲量的影响。方法:选取我院2020年8月至2022年5月102例前交叉韧带重建术后患者为研究对象,依照康复方式分为常规组和限制组(n=51)。常规组予康复常规训练,限制组在常规组基础上联合血流限制性运动训练,干预3 m后,取患者患侧髌骨上极6 cm处股四头肌的横断面核磁片,采用Image Pro Plus6.0图像分析软件测定股四头肌横截面积,采用美国Biodex System4等速肌力评估训练系统测定伸膝峰力矩(Peak torque,PT)、屈伸肌比值(Hamstring/quadriceps,H/Q)、相对峰力矩(Peak torque/body weight,PT/BW),采用2m3Dbox足底压力平板测试系统测定健、患侧足偏角与足跟、前足内外侧冲量百分比,统计满意度及下肢肿胀、慢性疼痛等并发症情况。结果:干预后患者PT、H/Q、PT/BW及足跟内外侧冲量百分比均明显升高(P<0.05),且限制组高于常规组;干预后患者股四头肌横截面积、前足内外侧冲量百分比及健、患侧足偏角均明显降低(P<0.05),且限制组股四头肌横截面积高于常规组,前足内外侧冲量百分比及患侧足偏角低于常规组(P<0.05);两组干预前后健侧足偏角经比较无差异(P>0.05);限制组满意率高于常规组(P<0.05);两组并发症发生率比较无显著差异(P>0.05)。结论:血流限制性运动训练可改善前交叉韧带重建术后患者足偏角,调节股四头肌肌力,改善足底冲量,提高满意度,有良好安全性。Objective:To investigate the effects of blood flow-restricted exercise training on quadriceps muscle strength,foot deflection,and planar impulse in patients with anterior cruciate ligament reconstruction.Method:A total of 102 patients with anterior cruciate ligament reconstruction in our hospital from August 2020 to May 2022 were selected as research objects,and divided into a conventional group and a restricted group(n=51)according to rehabilitation methods.The conventional group was given routine rehabilitation training,and the restricted group was combined with blood flow restriction exercise training on the basis of the conventional group.After 3 m of intervention,a cross-section of the quadriceps muscle at the upper pole of the patella on the affected side of the patient was taken,and the cross-section area of the quadriceps muscle was measured by Image-Pro Plus6.0 image analysis software.Peak torque(PT),Hamstring/quadriceps(H/Q),and Peak torque/body weight(PT/BW)were measured by Biodex System4 isometric muscle strength assessment system.2m3Dbox plantar pressure plate test system was used to measure the percentage of healthy and affected lateral foot deflection,heel,and forefoot inner and outer impulse,and to calculate satisfaction,lower limb swelling,chronic pain,and other complications.Results:After the intervention,PT,H/Q,PT/BW,and the percentage of impulse inside and outside the heel were significantly increased(P<0.05),and the restricted group was higher than the conventional group.After the intervention,the quadriceps cross-sectional area,medial and lateral impulse percentage of the forefoot,and deflection Angle of healthy and affected foot was significantly decreased(P<0.05),and the quadriceps cross-sectional area of the restricted group was higher than that of the conventional group,medial and lateral impulse percentage of forefoot and deflection Angle of affected foot were lower than that of the conventional group(P<0.05).There was no difference in the Angle of the healthy lateral foot between the t
