摘  要:目的:探究ω-3鱼油脂肪乳辅助营养支持应用于保留十二指肠胰头切除术患者的效果。方法:选取我科2020年7月至2022年9月期间104例保留十二指肠胰头切除术患者,根据抽签法分组。对照组52例给予胃肠外营养支持与肠内混合营养支持,观察组52例在此基础上给予添加ω-3鱼油脂肪乳辅助营养支持,干预6 d后对比两组患者术后胃肠功能恢复指标:观察患者术后排气时间、住院时间、下床活动时间;抽取清晨空腹静脉血分离血清,采用双缩脲法检测白蛋白(Albumin,ALB)、总蛋白(Total Protein,TP),使用全自动血细胞分析仪检测血红蛋白(Hemoglobin,Hb)水平等围手术期营养指标;检测白细胞计数(White blood cell count,WBC)、淋巴细胞计数(Lymphocyte,LY),采用免疫层析法检测降钙素原(Procalcitonin,PCT),酶联免疫吸附法检测白细胞介素6(Interleukin-6,IL-6)水平等了解围手术期炎症反应。结果:两组患者ALB、TP、Hb水平无统计学意义(P>0.05);观察组排气时间、下床活动时间、住院时间均短于对照组,干预后WBC、LY、PCT、IL-6水平低于对照组(P<0.05)。结论:添加ω-3鱼油脂肪乳辅助营养支持对保留十二指肠胰头切除术患者干预,可有效降低机体炎症反应,改善营养状态和胃肠功能恢复。Objective:To investigate the effect of omega-3 fish oil fat milk supplemental nutrition support in patients with preserved duodeno-pancreaticotomy.Methods:A total of 104 cases of reserved duodenopancreatic resection from July 2020 to September 2022 in our department were selected and grouped according to the lottery method.Control group 52 cases were given parenteral nutrition support and enteral mixed nutrition support,Observation group 52 cases were given the supplemental nutrition support of ω-3 fish oil fat milk on this basis.After 6 days of intervention,the indexes of postoperative gastrointestinal function recovery were compared between the two groups:postoperative exhaust time,length of hospital stay and time of getting out of bed were observed.Fasting venous blood serum was extracted in the morning,and perioperative nutritional indexes such as Albumin(ALB)and Total Protein(TP)were measured by biuret method,and Hemoglobin(Hb)levels were measured by automatic blood cell analyzer.White blood cell count(WBC)and Lymphocyte count(LY)were tested,Procalcitonin(PCT)was detected by immunochromatography.Interleukin-6(IL-6)levels were measured by enzymelinked immunosorbent assay to understand perioperative inflammatory responses.Results:There was no statistical significance in the levels of ALB,TP and Hb in 2 groups(P>0.05).The exhaust time,the time to get out of bed and the time to stay in hospital in the observation group were shorter than those in the control group,and the levels of WBC,LY,PCT and IL-6 after intervention were lower than those in the control group(P<0.05).Conclusion:Supplementation of omega-3 fish oil and fat milk with supplemental nutritional support can effectively reduce inflammation and improve nutritional status and gastrointestinal function recovery in patients undergoing duodenopancreatic resection.
