摘 要:目的探讨分析糖尿病肾脏疾病患者肠道菌群与免疫功能的关系。方法选取河南科技大学第一附属医院2021年6—12月收治的88例2型糖尿病患者,根据是否发生慢性肾病分为单纯糖尿病组(52例)和糖尿病肾脏疾病组(36例),并选取同期进行健康体检的40例志愿者作为对照组。入院清晨取其粪便样本进行肠道细菌检测,测定患者肠道菌群分布情况。并采集血清样本进行免疫功能指标检测,测定患者免疫球蛋白G(IgG)、免疫球蛋白A(IgA)、免疫球蛋白M(IgM)和CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平。采用Pearson相关性分析糖尿病肾脏疾病患者肠道菌群与免疫功能的关系。结果3组乳酸菌数、酵母菌数比较差异无统计学意义(P>0.05);糖尿病肾脏疾病组大肠杆菌数高于单纯糖尿病组和对照组,双歧杆菌数、真杆菌数低于单纯糖尿病组和对照组(P<0.05)。3组IgG、IgA、IgM、CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平,比较差异有统计学意义(P<0.05),糖尿病肾脏疾病组IgG、CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)高于单纯糖尿病组和对照组,而IgA、IgM低于单纯糖尿病组和对照组(P<0.05)。Pearson相关性分析结果显示,大肠杆菌数与IgG、CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)呈正相关,与IgA、IgM呈负相关(P<0.05);双歧杆菌数和真杆菌数与IgA、IgM呈正相关,与IgG、CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)呈负相关(P<0.05)。结论糖尿病肾脏疾病患者肠道菌群分布与免疫功能密切相关,肠道菌群分布紊乱,免疫功能下降,可能参与糖尿病肾脏疾病进展,可为疾病防治提供一定指导。Objective To investigate the relationship between intestinal flora and immune function in diabetic nephropathy patients.Methods A total of 88 patients with type 2 diabetes admitted to the First Affiliated Hospital of Henan University of Science and Technology from June to December 2021 were selected.According to the occurrence of chronic kidney disease,they were divided into simple diabetes group(52 cases)and diabetic nephropathy group(36 cases),and 40 volunteers who underwent physical examination during the same period were selected as the control group.Stool samples were collected in the morning of admission for intestinal bacteria detection to determine the distribution of intestinal flora of patients.Serum samples were collected for the detection of immune function indexes,and the patients’immunoglobulin G(IgG),immunoglobulin A(IgA),immunoglobulin M(IgM)and CD3^(+),CD4^(+),CD4^(+)/CD8^(+)levels were determined.Pearson correlation was used to analyze the relationship between intestinal flora and immune function in diabetic nephropathy patients.Results There was no statistical difference in the number of Lactobacillus and Yeast among the three groups(P>0.05).The number of Escherichia coli in diabetic nephropathy group was higher than that in simple diabetes group and healthy control group,and the number of Bifidobacterium and Eubacillus number were lower than that in simple diabetes group and healthy control group(P<0.05).The levels of IgG,IgA,IgM,CD3^(+),CD4^(+),CD4^(+)/CD8^(+)were statistical different among the three groups(P<0.05).The levels of IgG,CD3^(+),CD4^(+),CD4^(+)/CD8^(+)in diabetic nephropathy group were higher than those in simple diabetic group and healthy control group,and the IgA and IgM were lower than those in simple diabetes group and healthy control group(P<0.05).Pearson correlation analysis showed that the number of Escherichia coli was positively correlated with IgG,CD3^(+),CD4^(+),CD4^(+)/CD8^(+),and negatively correlated with IgA and IgM(P<0.05).The number of Bifidobacterium and Eubac
