摘　　要：目的探讨芪地桃蛭二蝉方联合小剂量糖皮质激素、他克莫司胶囊治疗膜性肾病的效果及对肾脏微循环的影响。方法选取2021年1月至2022年6月收治的80例膜性肾病患者为研究对象,按照随机数字表法将其分为对照组和观察组,各40例。对照组采用小剂量糖皮质激素联合他克莫司胶囊治疗,观察组在对照组基础上加用芪地桃蛭二蝉方治疗。比较两组的治疗效果。结果观察组的治疗总有效率高于对照组,差异具有统计学意义(P<0.05)。治疗前,两组的视黄醇结合蛋白(RBP)、N-乙酰-β-D-氨基葡萄糖苷酶(NAG)水平比较,差异无统计学意义(P>0.05);治疗后,观察组的RBP、NAG水平低于对照组,差异具有统计学意义(P<0.05)。治疗前,两组的舒张末期血流速度(EDV)、收缩期峰值流速(PSV)、阻力指数(RI)比较,差异无统计学意义(P>0.05);治疗后,观察组的EDV、PSV均高于对照组,RI低于对照组,差异具有统计学意义(P<0.05)。结论芪地桃蛭二蝉方联合小剂量糖皮质激素、他克莫司胶囊治疗膜性肾病的效果满意,能够有效降低肾损伤标志物水平,改善肾脏微循环。Objective To investigate the effect of Qidi Taozhi Erchan recipe combined with low-dose glucocorticoid and tacrolimus capsule on membranous nephropathy and its influence on renal microcirculation.Methods A total of 80 patients with membranous nephropathy admitted from January 2021 to June 2022 were selected as the research objects and divided into control group and observation group according to the random number table method,with 40 cases in each group.The control group was treated with low-dose glucocorticoid combined with tacrolimus capsules,and the observation group was treated with Qidi Taozhi Erchan recipe on the basis of the control group.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).Before treatment,there were no significant differences in the levels of retinol binding protein(RBP)and N-acetyl-β-D-glucosaminidase(NAG)between the two groups(P>0.05);after treatment,the levels of RBP and NAG in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Before treatment,there were no significant differences in end diastolic velocity(EDV),peak systolic velocity(PSV)and resistance index(RI)between the two groups(P>0.05);after treatment,the EDV and PSV in the observation group were higher than those in the control group,and the RI was lower than that in the control group,and the differences were statistically significant(P<0.05).Conclusion Qidi Taozhi Erchan recipe combined with low-dose glucocorticoid and tacrolimus capsule in the treatment of membranous nephropathy has a satisfactory effect,which can effectively reduce the level of renal injury markers and improve renal microcirculation.
