口服大黄水联合芒硝外敷腹部在结直肠手术患者术后胃肠功能恢复中的应用

摘　　要：目的探讨口服大黄水联合芒硝外敷腹部在结直肠手术患者术后胃肠功能恢复中的应用。方法选择2021年3月至2023年6月就诊于南昌大学第一附属医院高新医院的100例结直肠手术患者为研究对象,按抽签法分为试验组和对照组,各50例。对照组术后行常规西医对症处理,同时口服温开水。试验组术后口服等量生大黄水配合芒硝外敷腹部干预,比较两组手术指标、实验室指标和并发症发生情况。结果试验组腹胀缓解、肠鸣音恢复、肛门排气和术后住院时间短于对照组,住院费用少于对照组,差异有统计学意义(P<0.05);干预后,两组白细胞计数(WBC)、中性粒细胞(NE)百分比和C反应蛋白(CRP)水平低于干预前,且试验组各指标水平低于对照组,差异有统计学意义(P<0.05);干预后,两组IgA、IgM和IgG水平高于干预前,且试验组各指标水平高于对照组,差异有统计学意义(P<0.05);干预后,两组胃动素和促胃液素水平高于干预前,且试验组各指标水平高于对照组,差异有统计学意义(P<0.05);两组术后并发症发生率比较,差异无统计学意义(P>0.05)。结论在结直肠手术患者术后联合应用大黄水口服和芒硝外敷腹部可促进胃肠功能恢复,调节胃肠激素,提高免疫功能,安全性高。Objective To investigate the application of oral Rhubarb water combined with Mirabilite externally on abdominal application in postoperative gastrointestinal function recovery of colorectal surgery patients.Methods A total of 100 colorectal surgery patients admitted to Gaoxin Hospital,the First Affiliated Hospital of Nanchang University from March 2021 to June 2023 were selected as the study objects,and were divided into test group and control group by drawing lots,with 50 cases each.Control group received conventional Western medicine symptomatic treatment after operation,and oral warm water.The experimental group took the same amount of raw Rhubarb water and applied Mirabilite externally to the abdomen after the operation.The surgical indexes,laboratory indexes and complications of the two groups were compared.Results The remission of abdominal distension,recovery of bowel sound,anal exhaust and postoperative hospitalization time in experimental groups were shorter than those in control group,and the hospitalization cost was lower than that in control group,with statistical significance(P<0.05).After intervention,the white blood cell count(WBC),neutrophil(NE)percentage and C-reactive protein(CRP)level of the two groups were lower than before intervention,and the index levels of the test group were lower than that of the control group,with statistical significance(P<0.05).After intervention,the levels of IgA,IgM and IgG in two groups were higher than before intervention,and the levels of indexes in experimental group were higher than those in control group,with statistical significance(P<0.05).After intervention,the levels of motilin and gastrin in two groups were higher than before intervention,and the levels of indexes in experimental group were higher than those in control group,with statistical significance(P<0.05).There was no significant difference in the incidence of postoperative complications between the two groups(P>0.05).Conclusion The combined application of Rhubarb water and Mirabilite externally on
