舒筋洗药熏洗联合手法推拿对创伤性肘关节术后关节僵硬的影响

摘　　要：目的:探讨舒筋洗药熏洗联合手法推拿对创伤性肘关节术后关节僵硬的影响。方法:选取郑州市第七人民医院2019年1月至2021年12月诊治的创伤性肘关节术后关节僵硬患者86例,按照随机数字表法分为观察组和对照组,每组43例。对照组给予手法推拿治疗,观察组给予手法推拿联合舒筋洗药熏洗治疗。观察两组患者Mayo肘关节功能评分(Mayo Elbow-Performance Score, MEPS)、关节肿胀程度、疼痛视觉模拟评分法(visual analogue scale, VAS)、日常生活能力改善情况及临床疗效。结果:两组MEPS评分比较,观察组治疗后疼痛评分低于对照组,活动范围、稳定性和日常生活评分高于对照组,差异有统计学意义(P<0.05);观察组有效率为97.67%,对照组有效率为86.05%,观察组有效率高于对照组,差异有统计学意义(P<0.05);观察组治疗后消肿指数、VAS评分低于对照组,Barthel指数高于对照组,差异有统计学意义(P<0.05)。结论:舒筋洗药熏洗联合手法推拿可有效改善创伤性肘关节术后关节僵硬,缓解疼痛,提高肘关节活动功能,改善其日常生活能力,有利于肘关节功能恢复。Objective:To explore the influence of tendon-soothing drugs for fumigation and washing combined with manual massage on joint stiffness after traumatic elbow surgery.Methods:A total of 86 patients with joint stiffness after traumatic elbow surgery in The 7th People′s Hospital of Zhengzhou from January 2019 to December 2021 were selected and divided into the observation group and the con-trol group according to the random number table method,with 43 cases in each group.The control group was given manual massage treatment,while the observation group was given manual massage combined with tendon-soothing drugs for fumigation and washing.The Mayo elbow performance score(MEPS),joint swelling degree,visual analogue scale(VAS),daily life ability improvement and clinical efficacy of the two groups were observed.Results:In terms of the MEPS score of the two groups,the pain score of the observation group was lower than that of the control group after treatment,and the range of motion,stability and daily life score were higher than those of the control group,with statistically significant differences(P<0.05).The effective rate of the observation group was 97.67%,while that of the control group was 86.05%;the effective rate of the observation group was higher than that of the control group,with a statisti-cally significant difference(P<0.05).The detumescence index and VAS score of the observation group were lower than those of the control group,while the Barthel index was higher than that of the control group,with statistically significant differences(P<0.05).Conclusion:Tendon-soothing drugs for fumigation and washing combined with manual massage can effectively improve the joint stiffness after traumatic elbow surgery,relieve pain,improve the elbow joint activity function,enhance the ability of daily life,and is conducive to the recovery of elbow joint function.
