揿针围攻法联合穴位艾灸对下肢动脉硬化闭塞症患者疼痛的影响

摘　　要：目的:观察揿针围攻法联合穴位艾灸对下肢动脉硬化闭塞症(lower limb arteriosclerosis obliterans,LLASO)患者疼痛的影响。方法:选取2020年10月至2022年9月兴国县人民医院诊治的LLASO患者66例,按照随机数字表法法分为观察组和对照组各33例。两组患者均给予常规治疗,观察组另给予揿针围攻法联合穴位艾灸治疗。观察两组患者治疗前后疼痛视觉模拟评分(visual analogue score,VAS)、疼痛持续时间、焦虑自评量表(self-rating anxiety scale,SAS)评分、同型半胱氨酸(homocysteine,Hcy)、踝肱指数(ankle brachial index,ABI)、证候积分、小腿和足趾皮温变化情况及临床疗效。结果:观察组有效率为66.67%,对照组有效率为93.94%,观察组有效率高于对照组,差异有统计学意义(P<0.05);观察组治疗后VAS评分、SAS评分均低于对照组,患肢疼痛持续时间短于对照组,差异有统计学意义(P<0.05);观察组治疗后Hcy水平低于对照组,ABI、小腿皮温及足趾皮温高于对照组,差异具有统计学意义(P<0.05);观察组治疗后证候积分均低于对照组,差异具有统计学意义(P<0.05)。结论:揿针围攻法联合穴位艾灸能减轻LLASO患者疼痛,减少疼痛持续时间,改善下肢血液循环、焦虑情绪,提高ABI、小腿皮温、足趾皮温。Objective:To observe the influence of thumbtack needle siege combined with acupoint moxibustion on pain in patients with lower limb arteriosclerosis obliterans.Methods:A total of 66 LLASO patients who were treated in Xingguo People′s Hospital from Octo-ber 2020 to September 2022 were selected and divided into the observation group and the control group according to the random number table method,with 33 cases in each group.Both groups were given conventional treatment first,and then the observation group was addi-tionally given thumbtack needle siege combined with acupoint moxibustion.The visual analogue score(VAS),pain duration,self-rating anxiety scale(SAS),homocysteine(Hcy),ankle brachial index(ABI),syndrome score,calf and toe skin temperature changes and clinical efficacy of the two groups were observed before and after treatment.Results:The effective rate of the observation group was 66.67%,while that of the control group was 93.94%.The effective rate of the observation group was higher than that of the control group,with statistically significant difference(P<0.05).After treatment,the VAS score and SAS score of the observation group were lower than those of the control group,while the duration of limb pain was shorter than that of the control group,with statistically significant differences(P<0.05);The Hcy level in the observation group was lower than that in the control group,while the ABI,skin tem-perature of the calf and the toe were higher than those in the control group,with statistically significant differences(P<0.05);The syndrome scores in the observation group were lower than those in the control group,with statistically significant differences(P<0.05).Conclusion:Conclusion:The method of thumbtack needle siege combined with acupoint moxibustion can relieve the pain of pa-tients with LLASO,reduce the duration of pain,improve the blood circulation of lower limbs,anxiety,and increase the skin temperature of the ABI,calf and toe.
