四磨汤鼻饲对食管癌术后胃肠功能恢复的影响

摘　　要：目的 探究四磨汤经鼻空肠营养管鼻饲对食管癌术后胃肠功能恢复的影响。方法 选择2013年9月—2021年9月于福建中医药大学附属人民医院行食管癌手术的患者80例,采用随机数字表法分为研究组(40例)及对照组(40例)。所有患者均行食管癌根治术,术后均给予常规肠内及肠外的营养支持。研究组术后应用四磨汤鼻饲治疗,对照组术后经鼻空肠营养管注入温生理盐水,2组患者均连续治疗7 d。对比2组胃肠功能恢复情况、中医证候积分及胃泌素水平。结果 治疗后,研究组胃肠功能恢复时间短于对照组、各中医证候积分低于对照组、胃泌素水平高于对照组,差异均有统计学意义(P<0.05)。结论 食管癌手术患者术后经鼻空肠营养管鼻饲四磨汤有助于确保胃肠激素稳定、促进胃肠功能恢复、缩短术后恢复时间,值得推广应用。Objective To investigate the effect of Simo decoction via nasojejunal feeding tube on the recovery of gastrointestinal func⁃tion in patients with esophageal cancer after surgery.Methods A total of 80 patients who underwent esophageal cancer surgery in People’s Hospital Affiliated to Fujian University of Traditional Chinese Medicine from September 2013 to September 2021 were se⁃lected and divided into a study group(n=40)and a control group(n=40)by random number table method.All patients underwent radi⁃cal resection for esophageal cancer,and were given routine enteral and parenteral nutritional support after operation.The study group was treated with Simo decoction by nasal feeding after operation,and the control group was injected with warm normal saline through a nasojejunal feeding tube after operation.Both groups were treated continuously for 7 days.The recovery of gastrointestinal function,TCM syndrome scores and gastrin levels were compared between the two groups.Results After treatment,the gastrointestinal function recovery time of the study group was shorter than that of the control group,the scores of TCM syndrome were lower than that of the con⁃trol group,and gastrin level was higher than that of the control group,with statistical significance(P<0.05).Conclusion In patients with esophageal cancer after surgery,nasal feeding of Simo decoction through a nasojejunal feeding tube can help to ensure the stabil⁃ity of gastrointestinal hormones,promote the recovery of gastrointestinal function,and shorten the postoperative recovery time,which is worthy of popularization and application.
