基于数据挖掘中医药预防大肠息肉术后复发的临床研究

摘　　要：目的 通过挖掘中医药预防大肠息肉术后复发的用药规律及治疗方法,探讨大肠息肉发病机制,为结肠息肉的临床诊治提供依据。方法 整理2010年后国内公开发表的、具有明确疗效的中医药预防结肠息肉术后复发的文献,利用古今医案平台系统V 2.4.3版本进行数据统计分析与挖掘,总结相关用药规律。结果 共纳入相关文献50篇,预防大肠息肉复发中药组方50首,涉及中药173味;10次以上高频药10味,分别为茯苓、甘草、党参、白术、薏苡仁、陈皮、白花蛇舌草、乌梅、莪术、黄芪;15次以上关联药物5对,分别为党参-茯苓、白术-党参、白术-茯苓、陈皮-茯苓、薏苡仁-茯苓;得出方剂树包含以下药物:茯苓、党参、白术、陈皮、甘草、薏苡仁、山药;中药及方剂功效均以健脾、祛湿、清热、活血为主。结论 在大肠息肉发病中,脾虚为本,湿、热、瘀为标,其治疗原则以健脾祛湿为主,以清热解毒、活血化瘀为辅。Objective By exploring the medication rules and treatment methods of traditional Chinese medicine to prevent recurrence of colon polyp after operation,to discuss the pathogenesis of colon polyp,and provide the diagnosis and treatment basis for clinical treatment of colon polyp.Methods Sorting out the literatures on prevention of postoperative recurrence of colon polyps with definite curative effect published in China since 2010,the data were statistically analyzed and mined using V2.4.3 version of ancient and modern medical case platform system,and the relevant medication rules were summarized.Results A total of 50 related literatures and 50 Traditional Chinese Medicine prescriptions for preventing recurrence of colon polyps are included,there are 173 species of Traditional Chinese Medicine and ten of them are high frequency of occurrence,including poria cocus,glycyrrhiza,codonopsis,atrac‐tylodes atractylodes,coix seed,tangerine peel,oldenlandia diffusa roxb,dark plum.There are five pairs of fifteen times associated drugs,including codonopsis-poria,atractylodes macrocephala-codonopsis,atractylodes atractylodes-poria cocos,tangerine peel-poria cocos,coix seed-poria cocos.The formula tree contains the following drugs:poria cocos,codonopsis pilosula,atractylodes atrac‐tylodes,tangerine peel,licorice root,coix seed and yam.The effects of traditional Chinese medicine and prescriptions are mainly spleen strengthening,dispelling dampness,clearing heat and promoting blood circulation.Conclusion In the pathogenesis of colon polyp,spleen deficiency is the key point,and dampness,heat and stasis are the symptoms.The main treatment principles are invigo‐rating spleen and removing dampness,with heat clearing and detoxification,activating blood circulation and removing stasis as supplement.
