钩草平肝汤联合针刺治疗肝阳上亢型后循环缺血性眩晕临床观察

摘　　要：目的观察自拟钩草平肝汤联合针刺治疗肝阳上亢型后循环缺血性眩晕的临床效果。方法采用随机对照原则,将符合纳入标准的60例肝阳上亢型后循环缺血性眩晕患者分为对照组、观察组,各30例。对照组用盐酸倍他司汀注射液静脉滴注治疗,观察组在对照组基础上加自拟钩草平肝汤口服及针刺治疗,2组疗程均为10 d。比较2组患者的中医证候疗效、经颅多普勒(TCD)血流检查指标等。结果治疗后,观察组的总有效率为96.67%(29/30),高于对照组的76.67%(23/30),差异有统计学意义(P<0.05)。2组患者的TCD各项指标平均血流速度(Vm)较治疗前均有明显提高,且观察组的左、右椎动脉及基底动脉的Vm明显高于对照组,差异均有统计学意义(P<0.05)。结论自拟钩草平肝汤联合针刺治疗后循环缺血性眩晕效果显著,可明显改善患者的眩晕症状,值得临床进一步推广。Objective To observe the clinical efficacy of self-made Goucao Pinggan decoction combined with acupuncture in the treat⁃ment of posterior circulation ischemic vertigo of hyperactivity of liver yang type.Methods According to the principle of random con⁃trol,60 patients with posterior circulation ischemic vertigo of hyperactivity of liver yang type who met the inclusion criteria were ran⁃domly divided into the control group and the observation group,with 30 cases in each group.The control group was treated by intrave⁃nous infusion of betahistine hydrochloride injection,and the observation group was treated by oral administration of self-made Goucao Pinggan decoction combined with on the basis of the control group.The course of treatment of both groups was 10 days.The curative effect of TCM syndrome and transcranial Doppler(TCD)blood flow examination were compared between the two groups.Results After treatment,the total effective rate of the observation group was 96.67%(29/30),which was higher than that of the control group(76.67%,23/30),and the difference was statistically significant(P<0.05).The average blood flow velocity(Vm)of TCD indexes in 2 groups was significantly higher than before treatment,and the Vm of left and right vertebral artery and basilar artery in observation group was sig⁃nificantly higher than that in control group,and the difference was statistically significant(P<0.05).Conclusion Self-made Goucao Pinggan decoction combined with acupuncture has significant curative effect in the treatment of post-circulatory ischemic vertigo,and can significantly improve the vertigo symptoms of patients,which is worthy of further clinical promotion.
