龙牡壮骨颗粒联合阿法骨化醇治疗儿童佝偻病的疗效及对骨代谢、生长发育的影响

摘　　要：目的探究龙牡壮骨颗粒联合阿法骨化醇治疗儿童佝偻病的疗效,分析其对骨代谢、生长发育的影响。方法选取2020年1月至2023年1月郑州市金水区总医院70例佝偻病患儿为研究对象,依据随机单双数分配原则分为单一组、联合组,每组各35例。单一组予以阿法骨化醇治疗,联合组予以龙牡壮骨颗粒联合阿法骨化醇治疗。比较两组临床疗效,生长发育状况及治疗前后骨代谢[硬骨素(SOST)、I型前胶原氨基端原肽(PINP)、骨钙素(BGP)、Ⅰ型胶原交联羧基端肽(CTX-I)],血清生化指标[骨碱性磷酸酶(BALP)、25-羟维生素D_(3)[25(OH)D_(3)]、血磷、血钙],维生素D代谢相关指标[甲状旁腺素(PTH)、维生素D结合蛋白(VDBP)]水平。结果联合组总有效率高于单一组,生长发育状况优于单一组(P<0.05)。治疗后,联合组血清SOST、PINP、BGP、CTX-I水平高于单一组(P<0.05);联合组血清BALP、25(OH)D3、血磷、血钙水平高于单一组(P<0.05);联合组血清PTH水平低于单一组,VDBP水平高于单一组(P<0.05)。结论龙牡壮骨颗粒联合阿法骨化醇治疗儿童佝偻病的疗效显著,可改善骨代谢,促进患儿生长发育,并可纠正钙磷代谢、维生素D代谢紊乱状态。Objective To explore the curative effect of longmu zhuanggu granules combined with alfacalcidol in the treatment of rickets in children,and to analyze its effects on bone metabolism,growth and development.Methods 70 children with rickets in Jinshui District General Hospital of Zhengzhou City from January 2020 to January 2023 were selected as research objects,and were divided into single group and combined group according to the random odd-even number allocation principle,with 35 cases in each group.The single group was treated with alfacalcidol,and the combined group was treated with longmu zhuanggu granules combined with alfacalcidol.The clinical efficacy,growth and development status,bone metabolism[SOST],type I procollagen amino terminal propeptide(PINP),osteocalcin(BGP),type I collagen cross-linked carboxy-terminal peptide(CTX-I)],serum biochemical indexes[bone alkaline phosphatase(BALP),25-hydroxyvitamin D_(3)[25(OH)D_(3)]and blood phosphorus,blood calcium],vitamin D metabolism related indicators[parathyroid hormone(PTH),vitamin D binding protein(VDBP)]levels of the two groups were compared before and after treatment.Results The total effective rate of combined group was higher than that of single group,and the growth and development status was better than that of single group(P<0.05).After treatment,the serum levels of SOST,PINP,BGP and CTX-I in combination group were higher than those in single group(P<0.05);the serum levels of BALP,25(OH)D3,blood phosphorus and blood calcium in combination group were higher than those in single group(P<0.05);the serum PTH level of the combined group was lower than that of the single group,and the VDBP level was higher than that of the single group(P<0.05).Conclusion Longmu zhuanggu granules combined with alfacalcidol can improve bone metabolism,promote the growth and development of children with rickets,and correct the disorder of calcium and phosphorus metabolism and vitamin D metabolism.
