摘　　要：老年慢性病具有病症复杂、病程长、康复慢、发病隐匿的特点,故需为患者提供长期、高效的护理措施。连续护理将院内护理与社区护理、家庭护理综合并形成全程性一体化护理模式,充分保障了护理措施的连贯性,有助于改善患者症状,保障其身心健康与生活质量。该文围绕社区老年慢性病的特点、连续护理的概念及具体措施作一综述,以期为社区老年慢性病患者护理措施的制定提供一定参考。Chronic diseases in the elderly are characterized by complex symptoms,long course of disease,slow recovery,and insidious onset,so patients need to receive long-term efficient nursing measures.Continuum nursing integrates in-hospital nursing,community nursing,and home nursing to form a whole-process integrated nursing model,which fully guarantees the continuity of nursing measures,helps to improve patients'symptoms,and ensures their physical and mental health and quality of life.This article reviews the characteristics of chronic diseases in the community,the concept of continuous nursing and specific measures,aiming to provide some reference for the formulation of nursing measures for elderly patients with chronic diseases in the community.
